    Job overview

    An exciting opportunity has arisen on B14. The role of the HCA is to assist in maintaining the highest possible standard of care under the supervision of a Registered nurse. To maintain a tidy and welcoming environment for patients and relatives. The staff will be involved with many members of the MDT.

    B14 specialises in caring for Maxilo facial, ENT and Urology patients. Tracheostomies and laryngectomies.

    Main duties of the job

    The ward is a busy, modern unit with a professional but friendly team delivering high standards of care. We are looking for a highly enthusiastic, motivated and committed support worker to deliver high quality holistic nursing care to patients and to join our forward thinking and ever developing team. You should have good communication and interpersonal skills, enabling you to work effectively as part of the team and deal with complex sensitive information in a professional manner.

    Main duties of the job

    1. Provide good quality personalised care in accordance with agreed local and national
    2. Performs essential nursing care to a high quality standard which meets the individual needs of patients
    3. If required, help patients with personal hygiene, including care of hair, teeth and nails and under supervision assist with oral hygiene of conscious patients
    4. Assist with bathing in bed and in the bathroom, with turning and repositioning of patients, using mechanical aids and bed appliances if required
    5. Assist in the care of pressure areas and the care of the incontinent patient
    6. Achieve and maintain Core Skills Training, complete the Care Certificate and Healthcare Assistant Induction
    7. Demonstrates compassion by recognising and responding to individual needs in a warm, thoughtful, sensitive and helpful way
    8. Supports the delivery of high quality end of life care, including care of the deceased and bereaved relatives
    9. Maintains patient privacy, dignity with respect at all times
    10. Proactively engages with patients, relatives and carers to seek their views, and gain co-operation and agreement
    11. Reports or escalates concerns about any patient's condition immediately to a registered nurse
    12. Identify and report any patients needs that are not included in the care plan

    Detailed job description and main responsibilities

    13. Correct use, maintenance and cleaning of pressure relieving devices
    14. Provide patients with appropriate activities and stimulation to promote wellbeing and recovery in particular when providing enhanced care supervision
    15. Present a positive image of self, team and the organisation
    16. Introduces self to patients at the start of each shift
    17. Welcome visitors in a manner which promotes a positive image of yourself and team you work within
    18. Ensure compliance with Trust Uniform Policy
    19. Take samples from patients for testing or dispatching after appropriate training and competency assessments eg: Urine, sputum, faeces, MRSA swabs
    20. Ensure that any samples taken are dispatched as per Trust procedures
    21. Ensure that samples obtained are stored correctly while awaiting collection and take/send samples to pathology as directed.
    22. Complete all documentation correctly.
    23. Assist patients to meet nutritional and hydration needs in line with their individual care plan
    24. Ensure timely completion of menu card and gives assistance to patients if required
    25. Prepares patients for mealtimes in timely manner where required
    26. Assists patients with eating and drinking, provides adaptive aids if required.
    27. Prepares and administers beverages for patients in timely manner
    28. Weighs patients accurately as directed and records results
    29. Food/fluid charts are fully completed in a timely manner to Trust standards
    30. Assist to maintain patient safety, conforming to health, safety and security legislation, policies, procedures and guidelines .
    31. Uphold uniform policy at all times
    32. Ensure nurse call systems are answered in timely way
    33. Is able to demonstrate the correct use of cot slides, blue tray and wet floor signs.
    34. Is compliant with moving and handling update training
    35. Reports all accidents, incidents, near misses, faulty equipment, environmental issues immediately as per Trust Policy
    36. Assist in maintaining standards of care to improve the patient experience and outcome of
    37. Ensure patients are safe, comfortable with their care needs met
    38. Takes an active part in ward handovers
    39. Participates in audit and evaluation programmes as requested
    40. Makes constructive suggestions to how the services can be improved
    41. Challenges or reports poor standards of care or inappropriate behaviour and escalates concerns about the safety and wellbeing of patients
    42. Uses survey to gain patient feedback about their experience of their care
    43. Undertakes intentional rounding and documentation of as requested to meet ward standards
    44. In the absence of ward clerks participate in filing of notes, collection/delivery of notes from and to other departments. Admit onto PAS system and update EPTS if requested.
    45. Promote equality in care and practice by recognising, respecting and meeting the needs and choices of individuals
    46. Treats everyone with dignity and respect
    47. Offers choice to meet individual patients needs
    48. Acts in accordance with current legislation, policies and procedures and good practice relating to equality and diversity.

    51. Unpack linens and stores to maintain stock levels in a timely manner
    52. Ensure that ward and utility areas kept clean and tidy
    53. Make beds and change them in a timely manner to keep patients clean and comfortable and bed areas correctly prepared for new admissions and complete bedside checklist
    54. Carry out basic administration tasks in the absence of the ward clerk, eg filing, ordering, check stock levels, printing of patient wristbands
    55. Keep ward equipment cleaned and labelled, completing ward cleaning checklists eg commodes, baths and lockers
    56. Help patients with the care of their personal belongings
    57. Reports faulty equipment for repair promptly
    58. Orders and cancels pressure relieving equipment as directed
    59. Complete mattress audits checking mattresses are fit for purpose
    60. Dispose of clinical waste and used linen as per Trust Policy
    61. Ensure that telephones answered stating name, role and department within 6 rings where possible

    TRAINING AND DEVELOPMENT

    1. Maintain and develop own competence
    2. Actively participates in the appraisal process
    3. Has a current appraisal and Personal Development Plan
    4. Undertakes identified learning and development opportunities, as per Care Certificate and as agreed with manager.
    5. Mandatory training as outlined by the Trust is up to date
    6. Meets National HCA Code of Conduct (DH 2013)
    7. Ensure all learning and competencies are completed for use of any equipment used within your role
    8. Once airway competent and training completed and standards are maintained then progression to band 3.

    ORGANISATIONAL RESPONSIBILITIES

    1. Actively try to resolve issues and complaints at local level and then report the outcome to line manager
    2. Gives clear and concise advice to people on the procedures in place e.g PALS, complaints.
    3. Resolve potential conflicts as per conflict resolution guidance and the promotion of zero tolerance
    4. Report incidents of violence or aggression immediately in order to seek help and support

    PROFESSIONAL RESPONSIBILITIES

    1. Participate in the rehabilitation of patients in hospital
    2. Participate in the safe transfer of patients
    3. Support and encourage patients to be active participants in their care and recovery plans e.g. promoting independence with dressing, undressing, taking to the toilet and using commodes
    4. Transfer patients to other wards, clinics, departments or hospitals and undertake chaperon and escort duties in adherence with the Trusts Transfer Policy.

    PROCEDURES WHICH MUST NOT BE UNDERTAKEN BY A HEALTH CARE ASSISTANT

    1. Changing Intra-venous infusions
    2. Giving medical information to patients or relatives
    3. Gastric tube feeding
    4. Responsibility for the care of an unconscious patient
    5. Giving medicine or drugs – orally, by injection, inhalation or rectal administration
    6. Aseptic techniques
    7. Assessment of nursing needs, planning of care and evaluation of nursing care
