    Job summary

    We are looking for an enthusiastic, honest, self-motivated colleague who is able to provide high quality person centred care to patients, their families and carers. The Intensive Support Team provides care and treatment in the persons home to prevent hospital admissions to both the acute and mental health wards. The team works alongside the Specialist Dementia and Frailty Service in a range of settings in the West Essex Area.

    The service looks after patients living with dementia of any age and patients over the age of 75 with complex mental health needs.

    Main duties of the job

    The successful applicant will work as part of the Intensive Support Team Multi Disciplinary team which includes Drs, Nurses, OTs, associate practitioners.

    We work closely with our EPUT community specialist practitioners and Essex County Council colleagues, local organizations as well as the voluntary sector to support the holistic needs of the patient with a person centred collaborative approach.

    The successful candidate will be trustworthy, positive about change and will be passionate about the services we provide. The successful candidate will be able to work independently and autonomously, as well as part of a team therefore will need excellent verbal and written communication skills and will have excellent interpersonal skills and a zest for delivering the highest level of patient care and service delivery successful candidate will be able to adhere to the trust core values.

    Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

    All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.

    The successful candidate will have the opportunity to develop their knowledge and skills through working alongside our well established MDT and accessing specialist training.

    The West Essex area is a culturally diverse locality covering the beautiful Essex locality, the area covers both urban and rural geography.

    The post attracts a London fringe payment.

    A Car Driver is Essentialand the successful candidate must be able to travel throughout the West Essex locality covering HArlow, Uttlesford and Epping

  • Registration Mental Health Nurse RMN Part 3
  • Mentorship preparation
  • Evidence of continual professional development
  • Meet the requirements for revalidation as set out by the NMC
  • High standard of education
  • Desirable

  • Post graduate qualification or commitment to work towards this
  • Non-medical prescribing qualification
  • Knowledge

    Essential

  • Understanding of / and the ability to conduct assessments and review of care in line with the requirements of the mental health care clusters
  • Understanding and the ability to support the team to meet their key performance indicators and contractual targets
  • Experience in the performance management and supervision of lower banded staff
  • Excellent IT skills Word / Excel, PowerPoint and Outlook
  • Experience of using an electronic information system
  • Experience of and participation in audit and research
  • Knowledge and understanding of health legislation, principles of CPA and Care Management, Mental Capacity Act, Mental Health Act 1983 and safeguarding, the NHS Constitution, and risk assessment / management
  • Knowledge of current best practice and models within nursing practice in mental health
  • The ability to devise comprehensive care plans that incorporate effective, individualised and evidenced based therapy interventions
  • Understanding of the nature of mental health conditions and how they can affect people's day to day life and impact upon recovery

  • Proven experience of working in a health, social care or community setting as part of a multi- disciplinary team
  • Experience of dealing with confidential and sensitive information
  • Proven experience of working with people with mental health conditions and vulnerable service users
  • Proven experience of conducting and contributing to ongoing assessments
  • Ability to conduct risk assessments of the individual in the service user environment
  • Proven experience of medicines management in line with NMC Standards and Trust policies and procedures
  • Experience in providing psycho- social interventions
  • Demonstrate an understanding of the recovery principles
  • Ability to manage and coordinate mulit-disciplinary meetings referral / review / business / handover
  • Ability to work with Clinical Manager in the development and implementation of operational / Trust policies
  • Deliver training to colleagues and lower banded staff
  • Ability to critically analyse and review research findings and reports
  • Assess and treat own caseload of service users in accordance with Trust clinical pathways
  • Undertake physical health checks of services users and act on the findings appropriately
  • Undertake a range of clinical duties providing practical support and guidance to service users
  • Demonstrate an understanding of the Trust Core Values
  • Understand the need for compassion in working with colleagues, servicer users and families
  • Support families and carers within the boundaries of their role and NMC professional code of conduct
