- As a key contact for cases, you will be required to visit service users and their families/carers and undertake an assessment of need.
- You will be assessing for the provision of equipment to support individuals and help prevent, reduce and or delay care and maximise independence under the Care Act
- You will be assessing the need and outlining the specification for adaptations to homes within the eligibility of the Housing Grants, Construction and Regeneration Act .
- In the role, you'll refer issues if needed in accordance with Safeguarding procedures to protect vulnerable people. This will require you to maintain confidential reports to profession and Council guidelines.
- You'll be communicating, providing advice and guidance to service users and providers
- Daily you'll be liaising and working with Occupational Therapist, Social Workers, Senior practitioners and external contacts.
- It is essential that you have experience in a relevant area of statuary or voluntary sector work in health and social care.
- You must have a relevant qualification e.g. NVQ Level 3 in health or social care field
- We require you to have excellent interpersonal skills.
- You will be able to gather information from a variety of sources and follow assessment process.
- Ideally, you can demonstrate knowledge of relevant legislation, regulations and codes of practice; processes and procedures; support mechanisms and specialist equipment.
