    Job summary

    We are creating an Enhanced Care and Support team at the RUH. An exciting opportunity has arisen to join the team as an Enhanced Care Practitioner / RMN.

    The Enhanced Care and Support team will sit within the Vulnerable People Service. We are an ambitious service. We are recruiting to a Lead Nurse for Mental Health role. The Enhanced Care and Support Team will report to this position.

    The Enhanced Care Practitioner / RMN role has been designed to undertake assessment and provide direct care and support to people who require an enhanced level of care including activities, observation and supervision.

    Enhanced care and support will focus on person-centred care giving and holistic care planning. Enhanced Care Practitioners / RMNs will be an integral part of the team, delivering enhanced therapeutic care, support, and supervision to meet the physical and psychological wellbeing and additional needs of patients at the RUH.

    Excellent assessment skills and experience of supporting people with a range of mental health problems including complex clinical presentations is a requirement for the role.

    You will be required to work shifts including evenings, nights, weekends, and bank holidays.

    This rewarding position of Enhanced Care Practitioner / RMN will develop and evolve with our collaborative team working approach to ensure we meet the advancing demand of patients with additional needs.

    Main duties of the job

    Work alongside all the operational divisions to provide high quality holistic care to people with additional needs. This may include people with mental health conditions, people with eating disorders, people living with dementia, people with a learning disability and autistic people in the acute hospital setting.

    Support with the provision of bespoke 'on-the-job' coaching and training to all ward teams with regards to individualised and therapeutic interventions for patients who require enhanced care, role modelling to staff as part of their development.

    Role modelling and effective use of de-escalation techniques which may include communication, self-regulation, assessment, actions, and safety maintenance in order to reduce the risk of harm to patients, relatives/carers, visitors and staff.

    Administer medications as prescribed and maintain custody of all medicines, including controlled drugs, adhering at all times to the Trust's Policy on Administration of Medicines.

    Evaluate the effectiveness of interventions and update care plans to take account of the changing needs of the patient. Where appropriate plan the discharge needs of patients from admission, ensuring appropriate involvement of multi professionals and other agencies in the process.

    Be an advocate for the patient and ensure all their care needs are addressed in conjunction with the clinical team.

    Essential

  • Registered Mental Health Nurse
  • Evidence of ongoing continuous professional development
  • In-depth knowledge of the Mental Health Act (1983/2007), Mental Capacity Act and the Mental Capacity (Amendment) Act (2019), Equality Act (2010), Human Rights Act (1998)
  • Desirable

  • Post-registration qualification/post-graduate diploma and training preferably in Dementia/Delirium
  • Qualifications & Training / Knowledge & Experience

    Essential

  • Undertaken training in restrictive intervention, de-escalation skills, conflict resolution skills, and training regarding the risks of physical restraint and medical considerations
  • Knowledge of mental health conditions and their relationship to physical disorders to enable assessment /planning/ implementation and evaluation of care
  • Desirable

  • Experience of working in a dementia care setting, with people living with dementia
  • Experience of working with people with a learning disability and autistic people
