    Job Title: Clinical Pharmacist at LSMP & TLS PCN

    Job Summary: LSMP & TLS PCN is seeking a dynamic Clinical Pharmacist with previous experience in Primary Care to work alongside a team of professionals. This role offers the opportunity to directly impact patient care and contribute to service development within a supportive environment.

      Main Duties of the Job:
    • Autonomously support patients in managing long-term conditions and participate in vaccination programs.
    • Focus on the student and young professional population, helping them manage medication and prevent diseases.
    • Provide guidance on prescription and medication queries, policies, procedures, and safety initiatives.

    Job Responsibilities: Working closely with the pharmacy team, the Clinical Pharmacist will support prescription queries, contribute to service delivery, and assist in medicines optimization for patient safety and well-being.

    Person Specification:

      Qualifications
    • Essential:
      • Masters degree in pharmacy (MPharm) or equivalent.
      • Registration with General Pharmaceutical Council up to date.
    • Desirable:
      • Completion of relevant CPPE Primary Care Pathways.
      • Experience
    • Essential:
      • Demonstrable experience as a foundation-level pharmacist.
      • Ability to build relationships and work collaboratively across organizations.
      • Good clinical knowledge and evidence-based healthcare understanding.
    • Desirable:
      • Appreciation of general practice nature.
      • Previous PCN Clinical Pharmacist Experience.
      • Skills
    • Essential:
      • Ability to set and achieve targets in healthcare.
      • Rational prescribing strategies and understanding of health determinants.
      • Interpersonal, communication, and IT skills.
      • Effective risk management and teamwork.
      • Able to influence and persuade effectively.
    • Desirable:
      • Understanding of EMIS and SystmOne IT systems.
      • Personal Qualities
    • Essential:
      • Polite, confident, and cooperative.
      • Work well in a team and under pressure.
      • Integrity, communication skills, and self-awareness.
