    Paediatric Diabetes Specialist Nurse - Northampton, Northamptonshire, United Kingdom - Northampton General Hospital

    Description

    To be responsible for assessing, planning, implementation and evaluation of care to children of various ages and abilities with diabetes and within a philosophy of family centred care.

    To work as part of a multi-disciplinary team providing care for children/ young people living with diabetes.

    To liaise and communicate with networks within the acute trust, community and educational settings and regional networks and act as a source of specialist advice.

    To act as a source of specialist advice for children young people and families/carers on the nursing care and management of diabetes in the clinic, acute, home, educational and other care settings.

    Main duties of the job To be part of the Children's Diabetes multidisciplinary team delivering excellent evidence based practice. The role involves educating and supporting the children and their family from diagnosis, through transition to transfer to adult services.

    Being a resource for the families and educational facilities to ensure excellent management of diabetes.

    Working with the paediatric safeguarding team at NGH keeping the children and young people safe.

    To take part in educating the children, young people and education facilities.

    Capturing data for submission to audits.

    Supporting clinical Excellence

    Undertake induction and competency programmes appropriate to the role.

    Undertake specialist education training as required that will allow community nursing care to be delivered through new ways of working.

    Act as a clinical expert and use own knowledge to develop and enhance the development, skills and competence of others.

    Act as a clinical resource, mentor/assessor/supervisor/preceptor of learners and staff as required.

    Actively promote and participate in clinical supervision/mentorship or equivalent support mechanisms.

    Ensure that learners are provided with and encouraged to take advantage of learning outcomes that will enable them to meet their clinical placement outcomes and objectives.

    Accept delegated line management responsibilities of other team members actively contributing to performance review and the appraisal and personal development planning process within the knowledge and skills framework.

    To be able to identify and troubleshoot problems within the paediatric diabetes service with the aim of providing solutions

    Plan and deliver specialist education/ training for nurses and other professionals.

    Professional Responsibilities

    Ensure that self and all nursing staff practice at all times in accordance with the NMC Code of Professional Practice, raising any concerns with the Lead Nurse, and action as directed.

    Supporting Patient Excellence

    Promote, develop and implement patient and public involvement, leading to service improvement.

    Ensure that self and team members act at all times to provide a personalised service to patients in accordance with agreed standards and promote quality, diversity and rights.

    Identify child protection issues, which affect the childs right as an individual.

    To ensure that the child and their parents/carers have the necessary skills to enable them to manage their diabetes.

    Participate in a seven day on-call service for children and young people and their families.

    To work in partnership with the child and family to promote independence and normalisation.

    To act as the childs advocate, promoting their rights in accordance with national policy.

    To encourage, promote and facilitate a smooth transition from paediatric to adult services

    Quality Assurance and Monitoring

    Ensure the delivery of care to patients meets the standards determined by the standards for better health.

    Ensure that the highest standards of infection prevention and control are practiced at all times.

    Contribute to the development of Trust and directorate policies, procedures, standards and guidelines and ensure compliance of self and others.

    Participate in audits and the development and implementation of relevant action plans and implementation of changes as required.

    Participate in the self-assessment review process.

    Participate in research projects related to diabetes.

    To play an active part in service redesign, and reconfiguration projects affecting change and new ways of working.

    To act autonomously and make clinical decisions using evidence based practice within own area of expertise.

    To keep up to date with new treatments and regimes.

    Actively contribute to complex case, multidisciplinary team and network meetings

    To ensure that key drivers such as the NSF for children and Every Child Matters are central to future service development.

    To ensure that your clinical expertise is represented in forward planning systems eg Shaping the Future exercise.

    To ensure that NICE guidance and modern devices are factored into future care planning.

    To ensure that new ways of working offer financial efficiency

    People/Management Excellence

    To promote effective communication channels both within the department /directorate and Trust, and outside agencies.

    To manage a dedicated caseload of children / young people with diabetes independently without supervision.

    Co-ordinate and develop team activities to ensure the delivery of a cost effective, efficient and quality service.

    Keep the Head Nurse, Paediatrics informed of all the relevant issues affecting patient care e.g. risk, resources, staffing levels, skills and equipment needs.

    Be aware of budgetary responsibilities and limitations ensure resources are used effectively, assisting in managing resources within directorate budget.

    Be aware of requirements of the Paediatric Diabetes Best Practice Tariff.

    To establish and maintain effective communication in complex and demanding situations.

    Promote and facilitate an inter-agency, multidisciplinary approach to care.

    To be responsible for the assessment, planning, implementation and evaluation of programmes of evidence based care according to changing health needs and varying levels of complexity. This includes ensuring that the childs holistic needs are met and the impact of the childs care and condition is minimised and successfully integrated into their daily life

    To assess the childs condition and refer for medical opinion or admission to hospital when necessary.

    Provide advice, education, support and information to parents/carers as required and involve parents/carers and the childs family in the planning and delivery of care.

    Establish and maintain effective communication with parents/carers.

    Recognise and respond appropriately to urgent and emergency situations

    Receive reports from team members on progress and changes to individuals clinical condition.

    Facilitate effective communication and liaison with other disciplines as necessary to meet the needs of the child and family especially relating to care and discharge arrangements.

    Ensure fully accurate and contemporaneous nursing records are written and maintained for each child/family.

    Actively participate in the development and implementation of new ways of working, incorporating research-based practice in order to enhance ongoing care.

    Act as a professional and clinical role model at all times.

    Maximise use of clinical and staff resources, implementing best practice in admission and discharge.

    The post holder may be required to carry out other relevant duties as required.

    • Qualified Nurse, RN Child.
    • Evidence of Continued Professional Development.
    • Degree, or equivalent level of education or training, or working towards degree level education.
    Desirable
    • Teaching and assessing qualification.
    • Prescribing course.
    Knowledge and Experience Essential
    • Proven Post registration experience in caring for children with diabetes, either in a ward or community setting.
    • Experience of prioritising caseloads.
    • Evidence of autonomous working.
    • An understanding of audit and research based practice.
    • Demonstrates knowledge of current issues in nursing and children's diabetes care.
    • Relevant paediatric diabetes qualification or willing to undertake.
    Desirable
    • Experience of working in a community setting.
    • Awareness of managing change.
    Skills Essential
    • Demonstrates ability to work in a multidisciplinary team.
    • Evidence of detailed understanding of safe guarding issues.
    • Sound teaching skills.
