An exciting opportunity has arisen for a suitable trained and experienced Band 7 practitioner to lead on Transition of Care from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS)


The post holder would be employed by Mersey Care and hold honorary contract status with Alder Hey Children's NHS Foundation Trust.


An interest in and good understanding in young people with mental health, learning disabilities and/ or neurodevelopmental disabilities and the key issues affecting Young people and their families during the transition process is essential.


The post holder will:


  • Be dynamic and experienced in working across a range of services utilising co production and collaborative working
  • Be responsible for the implementation and monitoring of national and local policies, including NICE and other good practice documents working in collaboration and partnership with children and adult services and key agencies to provide a seamless transition for children young people and their families.
  • Working alongside young people, carers and professionals across children and adult service lines to assist them plan and complete transition.
  • Liaise with other agencies and general hospitals around transitions e.g. Local Authority, Education and the 3rdsector.
  • Deliver awareness raising and training packages to help staff negotiate complex issues in transition.


A willingness to work flexibly and innovatively to develop the role as new initiatives within the trust develop access the local area.

The ability to travel between the service locations is a requirement for this post.

The post holder will attend and participate in transition of care meetings between Alder Hey and Mersey Care. The post holder will support practitioners in CAMHS and Adult mental health and learning disability services with transition planning. The post holder will liaise with other agencies around transitions e.g. YPAS, YPC and will lead training on transitions policy jointly within Mersey Care and Alder Hey. To work alongside the Transitions team and leads to develop models for
future transitions developments e.g. models of care, research bids.
To manage a small clinical case load as part of the transition from Alder Hey CAMHS to Mersey care.
