An exciting 6 month opportunity has arisen to be part of the Families and Integrated Community Care Division supporting the adult community nursing and therapy teams.

The aim is to support the development of the neighbourhood care teams and to drive integrated working.


You will be part of a friendly supportive management team who work hard to maintain a patient focus in everything we do.

We are seeking motivated, proactive individuals that help move services forward in partnership with primary care, secondary care, and our social care colleagues.

The post holder will be responsible for the operational management of identified services within a locality.

Effectively managing all services for which the post holder has responsibility, within the delegated resources available.

Working with Head of Locality to actively move towards the integration of services.


Supporting and enabling staff to innovate and improve services to meet the needs and preferences of patients whilst ensuring provision of seamless care along care pathways.

Ensure effective communication across the locality and with the Head of Locality.

Blackpool Teaching Hospitals NHS Foundation Trust is ideally situated just a forty five minute drive from Manchester.

The Trust provides services to the 440,000 residents of Blackpool, Fylde & Wyre and North Lancashire, as well as specialist tertiary care for Cardiac and Haematology patients.

DUTIES AND RESPONSIBILITIES


Support the Head of Locality in ensuring that services are delivered to high standards in accordance with national / local policies and guidance.

Ensure that services for which the post holder has responsibility are delivered in accordance with service specifications.


Manage capacity and demand in relation to services for which the post holder has responsibility and advise the Head of Locality on the configuration of service provision to achieve this.


Ensure that, within the resources available, the services for which the post holder has responsibility have a workforce who is appropriately trained and have the appropriate skill mix.

Contribute to the development of workforce planning within the locality in conjunction with HR representative and Head of Locality.


Implement and monitor risk management and quality assurance policies and systems effectively, ensuring a pro-active approach to the reduction of risk, clinical effectiveness and health and safety in line with Governance strategy, CQC, CQUIN requirements.

Take corrective action necessary to improve standards or performance.

Supported by the Head of Locality, manage issues and complaints raised by patients complying with the organisations complaints processes.


Conduct performance reviews of services, for which the post holder has responsibility, against specified NHS targets, and contractual key performance indicators.

Demonstrate strong leadership to staff for which the post holder has responsibility with particular reference to motivation and morale.

Contribute to the development and marketing of services within the locality.

Contribute to the development and implementation of relevant multi agency policies, protocols and procedures

Participate in Divisional on call rota.

Provide operational leadership to the staff for which the post holder has responsibility

Encourage and facilitate the development and maintenance of flexible clinical practice in relation to identified services with Head of Locality

Contribute to the improvement and maintenance of good working relationships, and effective joint working, between partner organisations relating to clinical area

Provide profession specific advice when required in conjunction with the Head of Locality and the Associate Director of Nursing.

In particular, promote, facilitate and direct the development of clinical audit and the management of clinical risk.
