- Clinically examine and assess patient needs from a physiological and psychological perspective & plan clinical care;
- Assess diagnosis, plan, implement & evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose & manage treatment plans for patients with or at risk of developing a long-term condition;
- Diagnose & manage both acute & chronic conditions, integrating both treatment methods into a management plan;
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national & practice protocols & within scope of practice;

- Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry. Canvey Island Primary Care Network has a fantastic opportunity for an enthusiastic and committed nurse practitioner to join our PCN on a full-time basis, for hours per week, to work across all six practices within our network. There is flexibility to discuss hours at interview.
The six practices within our PCN are: Oaklands Surgery, Third Avenue Doctors Surgery, The Island Surgery, The Surgery - Hawkesbury Road, The Community Practice and Canvey Village Surgery.
The main aim of this role is to increase health and wellbeing of patients through improved screening, identification, and disease management. This will entail a mixture of clinics, ward rounds, outreach events and home visits, as directed.
