    We are looking to recruit a dynamic, self-motivated Occupational Therapists to join our skilled, and highly motivated Occupational Therapist to work within the Ageing Well Broxtowe Community Team. The aim of the team is to ensure people remain safe and live independently at home, by using a strength-based approach to deliver community-based health and social care services.

    You will be working with adults aged 65 years or people with working age dementia who live in the Broxtowe locality who have complex physical needs and long-term conditions. This can also include co-morbidities, functional and organic mental health diagnoses, and substance misuse. This is an exciting opportunity to be part of an ever-changing community team.

    Broxtowe Community Team have excellent relationships with our key health colleagues including Community nursing service, Specialist nurses, Therapy staff and General Practitioners, enabling greater information sharing and members of the public receive a quality holistic assessment from all practitioners. Our practice is based around the principles of strengths-based approach and integrated working with health to deliver the right intervention at the right time in the right place.

    You will have the responsibility for the assessment and provision of a wide range of services, including equipment, moving and handling, rehousing and major adaptations. You will also work in collaboration with our wider team, our local housing providers, equipment providers, our health colleagues, and any other relevant agencies.

    Nottinghamshire County Council recognises the essential contribution which Occupational Therapists make towards preventing and reducing the ongoing support needs of people referred for personal care.

    You will be expected to: -

    · Manage a complex caseload including moving and handling and assist colleagues with safeguarding decision work.

    · Work in a strength-based way keeping people at the centre of our agenda and are committed to promoting independence and achieving peoples desired outcomes.

    · Support managers to meet the departments' performance and service delivery requirements, to manage systems effectively and to work positively within a culture of change.

    · The ability to be resilient and effective in supporting staff and managers in the current challenging financial climate.

    · Work autonomously and as part of a busy community team, alongside working with other disciplines and health, good working knowledge of relevant legislation and departmental policies.

    · Have effective communication, organisational and IT skills are essential.

    · You must also be able to work on your own initiative and meet deadlines whilst working under pressure and have an awareness of and a commitment to Equality and Diversity, Health, and Safety policy/procedures.

    · Knowledge of specialist equipment, DFG processes and complex moving & handling is desirable.

    An enthusiasm for Occupational Therapy, an inquisitive attitude, and being comfortable with learning by doing, will be key to success in this position.
