    This post involves regular, direct unsupervised access to vulnerable adults and therefore is exempt from the Rehabilitation of Offenders Act 1974

    East Lancashire Hospice is a registered charity, founded in 1984. We support and care for patients with a life limiting illness, their families and others close to them. at the hospice and in people's own homes across our community of Blackburn, Darwen, Hyndburn and the Ribble Valley.

    Senior Nurse – Community Supportive Care (Hospice at Home)

    An exciting opportunity has arisen as part of new investment and ongoing service development for a motivated individual passionate about high quality palliative and end of life care to join our team.

    You will have a key role in providing support to patients in their own home environment who have palliative and end of life care needs, role-modelling high quality care and clinical standards and promoting a learning environment for team member and other involved in the person's care whilst supporting informal carers.

    You will be responsible for planning, assessing, implementing, and evaluating care in collaboration with the District Nurses Team in order to prevent avoidable hospital admission or support.

    Are you able to assess, plan and evaluate the delivery of high quality care?
    Are you able to work in a community setting with a sound understanding of the support needed to help people to remain in the own home environment where this is possible?
    Are you passionate about palliative and end of life care?
    A salary range [dependant on qualifications and experience] of £35,603 – 39,956 [pro-rata for part-time hours].

    Hours are worked on a rota basis of up to any 5 days of the 7 day week to meet service need inclusive of weekends and Bank Holidays.

    We have an excellent, collaborative working environment with a generous holiday allowance of 27 days + Bank Holidays with the option to buy additional days.
