    An exciting opportunity has arisen for the right person to join the Dementia and Delirium team for a 7 month secondment post at Barts Health NHS based at Royal London are seeking to recruit a Dementia Specialist Nurse/Therapist who will work alongside the Dementia Clinical Nurse Specialists, Dementia Support Worker and the wider Dementia and Delirium Team to support the delivery of the Trust Dementia Strategy and improve patient is an exciting opportunity to improve the lives, care and experience of those living with dementia or delirium and their carers, while they are inpatients in our are looking for a nurse with experience of working with older people, who is enthusiastic, innovative, forward thinking and willing to go the extra mile to improve the care of people with dementia. You must portray excellent leadership skills and supervise junior staff members in the team. You will be required to deputise in the absence of the band of this role will be identifying patients with dementia and/or delirium to prompt appropriate referral and follow up after they leave hospital. The successful candidate will also work closely with the multidisciplinary teams on the wards to ensure person-centred care is being delivered. This will require you to have good communication and interpersonal skills.

    Main duties of the job

    The post holder will work with the dementia teams across the Trust to support the implementation and evaluation of a seamless Dementia care pathway, ensuring that patients receive the highest standard of clinical care, treatment & support throughout the hospital journey and beyond. They will specialise in the care of people with dementia & delirium & will portray a willingness to develop expertise in the provision of specialist advice & support to ward teams, people with dementia & their carers. They will play a key role in the delivery of the trust Dementia education programme, providing training & evaluating training programmes. They will participate in clinical audits & research within the specialist area.

    Together with the dementia teams, the post holder will contribute to the embedding of KPI targets & NICE Quality standards across the Trust, & the implementation of objectives within local & National Dementia Strategies.

    The post holder will influence the ongoing development of clinical practice within Dementia and delirium care practice, participate in research and contribute to the development of standards of care within the service, including the development of policies, protocols, guidelines and service developments in collaboration with multi- disciplinary colleagues.

  • Registered Nurse - General or Mental Health, Registered Occupational Therapist.
