Salary:
Up to £25,000 per annum depending on experience.
HCL Care Services are one of the leading providers of Adult Social Care in the Liverpool City Region.
We take pride in providing high standards of personalised and professional care to our customers in their own homes, to enable them to live as independently for as long as possible.
Due to an increase in demand we are recruiting for a Care Coordinator. You will be responsible for front line care staff within a designated locality.You will ensure that a safe and reliable service is delivered on behalf of HCL, which promotes best practice and puts the customer at the forefront of the service provision.
The role will require you to:
- Ensure care is provided to our customers according to all relevant policies, procedures and regulations.
- Allocating the right carer to the right customer at the right time.
- Maintain excellent and accurate communication between the workforce and management.
- Provide excellent customer service levels to all internal and external users of the business.
- Liaising with a variety of healthcare professionals, social workers and case managers
- Ensure compliance with all office systems and procedures.
- Take ownership of area and to support team within all daily aspect of the role.
Requirements:
- Previous experience in coordination.
- Sign up with support to obtaining a Level 3 QCF in Health & Social Care.
- Hold a current Full UK Driving licence and access to your own vehicle.
- Good IT skills.
- Planning and organisation.
