    Job summary

    An exciting opportunity has arisen for an experienced pharmacist to join our pharmacy team at Morriston Hospital working across both palliative care and medicine. In this unique role, you will have the opportunity to provide expert pharmaceutical care to patients across two distinct specialities.

    Your role will encompass a diverse range of responsibilities working closely with the multi-disciplinary team to optimise medication therapy, improve treatment outcomes and enhance the quality of life for our patients.

    Main duties of the job

    As the Pharmacist specialising in palliative care and medicine, you will:

  • Provide day-to-day leadership and management of the pharmacy service to both palliative care and medicine.
  • Collaborate with clinicians, nursing staff, pharmacy colleagues, and other multidisciplinary team members to deliver high-quality pharmaceutical care.
  • Offer ward-based medicines management services from admission to discharge, ensuring safe and effective medication use.
  • Proactively provide evidence-based advice to prescribers to enhance patient care and promote cost-effective medicine usage.
  • Engage in core directorate activities, including medicines expenditure analysis, clinical governance audits, and risk management.
  • Supervise, mentor, and train junior colleagues to support their professional development.
  • Participate in dispensary duties and provide clinical leadership to junior pharmacists and trainees.
  • Participate in weekend, Bank Holiday and out of hours rotas as dictated by the needs of the service.
  • Proficiency in Welsh is desirable for this role, but English and/or Welsh speakers are equally welcome to apply.

    Job responsibilities

    The ability to speak Welsh is desirable for this post. English and/or Welsh speakers are equally welcome to apply.

    To contribute to the ongoing operational provision of the pharmacy service, in line with departmental rotas to meet the needs of the service. Participate in weekend, Bank Holiday and out of hours rotas as dictated by the needs of the service

    Provide clinical pharmacy support to the dispensary as part of the pharmacist's dispensary rota. To communicate and work with locality teams to deliver an integrated service across the health community.

    Qualifications

    Essential

  • Post Graduate MSc/ Diploma in Clinical Pharmacy or equivalent experience
  • Member of the GPhC
  • Desirable

  • Non medical prescriber
  • Accredited Tutor for clinical pharmacy diploma
  • Experience

    Essential

  • Ability to undertake autonomous practice.
  • Experience as clinical pharmacist in specialist area.
  • Evidence of clinical audit, practise research, implementation of procedures, protocols.
  • Desirable

  • Lecture on specialist subject.
  • Experience of being a preregistration pharmacist tutor.
  • Aptitude & Abilities
