    TheCommunity Nursing Team provide nursing care to patients in their own homes inthe beautiful setting of west Devon.

    The Community Nursing Servicesits within a health and social care team so the work involves working with anintegrated approach to care, making every contact count.

    The team work closelywith GP partners, local Hospice's and Voluntary Partnerships. This is a great opportunity to gainskills working in the Community Nursing Service.

    This rolemay not be eligible for sponsorship under the Skilled Worker route, pleaserefer to the Direct Gov website for more information with regards toeligibility.

    The main duties of the role areto provide nursing care for housebound patients in their own homes.

    To promoteself care and preventative approaches to ill health with focus on wellbeing theemphasis being of preventing hospital admissions and living well.

    Provide holistic evidence based / best practice care to patients in accordance with National and Organisational approved policies/procedures and individual care plans.

    Be responsible for the assessing, triaging, planning, implementation and evaluation of care packages on a day-to-day basis with support from the Caseload Holder and in her/his absence.

    Be involved in referrals, triaging, discharge planning, and case conferences to support or manage appropriate care packages for patients.
    Maintain accurate records, which are confidential, up-to-date, and legible and that all care given has to be documented.

    These records may be paper or IT based system and must be maintained as specified in the Organisation Policies, NMC guidelines and Government directives.

    Undertake comprehensive risk assessments of all situations associated with the care of patients in order to ensure nurses and carers safety.

    Monitor and maintain the health, safety and security of self and others.

    The post holder is required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager; (if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations/ Chief Executive office).

    Be conversant with the vulnerable adult and child protection procedures and policies of Plymouth Community Healthcare.

    To act as a clinical advisor on health care issues within their area of expertise and act as a Link Nurse.

    Undertake health promotion as requested to all age groups and contribute to planned health promotion activities.
    Be aware of organisational, professional and national developments within primary care and work within these guidelines e.g. NSF/ NICE/ NMC.
    Be aware of and abide by LWSW approved Policies, Patient Group Directives, Standards and Quality Assurance Initiatives.

    Be responsible in the absence of the Caseload Holder to delegate nursing care to appropriately skilled staff and ensure that they are supported.

    Contribute to national and local policy development.
    Ensure that all loans of equipment are monitored and maintained in good working order as per operational policy.
    Complete on time and submit all necessary forms, documentation, including IT data and forward as required by the manager/organisation.

    The post holder will be required to use a computer, being responsible for timely recording patient activities for IT data collection, dealing with e-mail queries, stock and equipment ordering.

    To preserve confidentiality and comply with of the Data Protection Act, Access to Health Records and Consent for Treatment.
    Have a flexible approach to the working day in order to meet the needs of the patient/service.
    Report to line manager any untoward incidents e.g.

    complaints, accidents, defective equipment and take appropriate action to ensure safety of patients, relatives, self and colleagues and follow up with appropriate documentation.

    Report and record all incidents and near misses relating to health, safety, security, fire, physical violence, aggression and verbal abuse.

    Develop and maintain effective and appropriate communication networks with other members of the Primary Health Care Team and other agencies.

    Provide and receive complex, sensitive and confidential information and over come potential barriers to communication, such as language and disability.

    To communicate the role and eligibility criteria of the District Nursing Service.

    If the post holder is supplied with a mobile phone and a lone worker device they are for work purposes and are expected to abide by Organisation.

    Works within the NMC Code.

    In line with the annual development plan the post holder will be expected to undertake any training or development required to fulfil their role.


    Education & Training:


    Maintain own professional expertise by attending meetings, study days and in-service training and learn new specialist skills required for service development.

    Attend and participate in working groups and research projects as required.
    Participate in Reflective Practice, Clinical Supervision and Appraisals as per Trust policies.
    Assess, teach, support, and supervise colleagues including pre- registration nursing students and medical students.
    Take responsibility for own personal professional development and education.
    Work towards becoming a Nurse Prescriber, if appropriate, and prescribe within the organisation policy and NMC guidelines. Experience of working in a nursing team

    Band 5 Nurse

    The post holder will be delivering nursing care to include, wound care, medication administration, bowel care, catheter care, Complex Wound Management of end of life care, managing frailty as part of the MDT approach in preventative care.

    Using an recognised Nursing Model to assess, plan, implement, evaluate nursing needs of the patient. Its an exciting role with training offered
    Degree level qualification - NMC Registration is essential (Nursing students about to qualify may apply)
    As the role requires traveling around the rural community own transport is essential car/bike.
    Phlebotomy skills Wound Care Care of the end of life patient Care of Syringe Drivers Catheter Care Bowel Care Medication administration Able to use IT packages including email and Microsoft Excel Caring for patients with lower limb ulceration
