    Job Summary

    Linthorpe Surgery is seeking a dynamic and driven Care Navigation Hub Manager with strong leadership and interpersonal skills, and a knack for organizing tasks efficiently, while maintaining a deep commitment to providing top-notch patient care.

    The role involves working closely with our current Management Team, each member having specific roles and duties.

    Prior experience in a GP practice is a must.

    Main Duties of the Job:

    • People Management
    • Handling Significant Events and Complaints
    • Monitoring demand and capacity
    • Supervising day-to-day operational processes in the hub

    Linthorpe Surgery stands out as the largest Middlesbrough-based practice, boasting a team of 9 GP partners, 4 salariedGPs, 2 Clinical Practitioners, 4 Practice Nurses, 2 HCAs, and a dedicated administrative and reception staff. We are conveniently located near the town center, with a branch surgery in North Ormesby, catering to a patient list of over 20,000 individuals.

    Belonging to the Holgate Primary Care Network, we collaborate with social prescribers, pharmacists, enhanced care home practitioners, mental health practitioners, safeguarding lead, and care coordinators.

    Job Description

    Job Responsibilities

    Aspects of Role:

    (i) DAILY

    • Morning staff briefing for updates, daily focus, challenges, and reminders
    • Reviewing and planning for the day's appointments
    • Coordination with the GP on appointment availability
    • Managing team tasks and priorities
    • Adapting to daily pressures and ensuring team support
    • Organizing break times and seating arrangements efficiently

    PHONE LINE:

    • Monitoring and managing incoming calls
    • Regularly checking call volumes

    CAPACITY MANAGEMENT (APPOINTMENTS):

    • Reviewing and utilizing available appointments effectively
    • Converting unused appointments to same-day bookings
    • Ensuring all appointments are utilized and managing cancellations
    • Optimizing different types of appointments
    • Monitoring and optimizing postnatal and interpreter appointments

    MANAGE HOME VISITS:

    • Liaison and allocation of home visits
    • Adjusting home visit slots for same-day appointments where necessary

    WAITING LISTS:

    • Regularly reviewing waiting lists for any issues
    • Providing support and guidance to the booking team

    CALL-CARE TEAM (Manchester):

    • Effective communication with the remote Call Care Team
    • Addressing patient queries promptly and efficiently
    • Monitoring patient interactions and providing necessary support

    (ii) WEEKLY

    • Individual check-ins with staff members
    • Reviewing upcoming challenges and adjustments
    • Generating standard reports for appointments data

    (iii) MONTHLY

    • Planning rotas in advance
    • Performance reviews and staff inductions
    • Summarizing reports and identifying trends for management
    • Mandatory training sessions within the monthly schedule
    • Updating resources and scheduling team meetings regularly

    Person Specification

    Experience

    Essential

    • NHS/Primary Care General Practice background
    • Experience in workforce planning and rota management

    Desirable

    • Leading multidisciplinary teams experience
    • Performance management skills
    • Agenda and minutes preparation for meetings
    • Reception management experience in a GP practice
    • Managing patient complaints

    Qualifications

    Essential

    • Strong educational background with excellent literacy and numeracy

    Desirable

    • Leadership or Management Qualification, or commitment to working towards one
