Description

Under supervision of trained staff the post holder will be responsible for screening / assessing new patients that attend the wards at UHCW.

Provide specific functional assessments and case manage individual patients, contributing to effective discharges, ensuring adherence to Trust policy at all times.




*Discharge Team Assistant Practitioner - Full time. The successful applicant starts as a band 3 and can progress onto band 4 once they have completed all in-house competencies months) with the aim that you will be released to work towards the assistant practitioner qualification and/or complete in house competencies to progress onto a band 5 position.

We are looking for an enthusiastic, pro-active individual to join our team. The Discharge Team will provide you with the opportunity to develop your knowledge and skills within this specialist area.

You will work within an inter-disciplinary team, completing holistic assessments and liaising with the Multi disciplinary team to coordinate safe, timely discharges.

The team work collaboratively with community services and specialist services both within the hospital and in the community to ensure a patient's needs are met within the most appropriate community environment.

This post is based at UHCW site, but the post holder will be required to work across both sites to meet service demands as required.

You will attend board rounds and case manage multiple cases within your agreed level of competence for your banding. You will need to be able to organise and prioritise your time and case load effectively. You will need excellent communication and interpersonal skills and be able to work well under pressure.
