The Out of Hours Community Nursing Team across Cambridgeshire is currently offering opportunities for permanent contracts working full time & part time within CPFT across the county working in Cambridge Huntingdon, Peterborough .
We provide a service 7 days a week, working closely with other community services and PCN's to provide high standard holistic care to the population of Cambridgeshire .
All staff will be expected to work the following shift patterns to support the service over a 7 day period
Twilight Shift
Night shift :15
You will have the opportunity to develop yourself, with extensive learning and development opportunities.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.
As a Community Staff Nurse, you will be working as an autonomous practitioner within the out of hours countywide team.
You will provide high standard of nursing care and interventions within the patient's own home, with support from team leaders.
The patient will be at the centre of the care being provided, ensuring patient and carer participation in decision making.
You will be accountable for assessing, interpreting, planning, implementing, and evaluating treatment to patients within professional guidelines. Patient needs will be identified through holistic nursing.
You will have opportunity to provide support and education to peers, new staff and students.
