About us
Manor Farm Medical Centre is situated in a busy market town. We have 7700 patients registered with us who are serviced by our main surgery in Swaffham, and a branch surgery in Necton.
We are a forward-thinking dispensing, research, and training practice with 2 full time and 1 part time GP Partners, a part time salaried GP and a locum GP. We have a dedicated team working at the surgery, providing a comprehensive service to our practice population. The wider clinical team comprises of 1 Paramedic, 1 ANP, 3 Practice Nurses, 1 Nurse Associate, 2 highly skilled HCAs and a Phlebotomist. Dispensaryand Reception staff provide a point of contact for patients and act as a focalpoint of communication between patients, doctors, and other medical staff.
Our Administration team is busy behind the scenes ensuring referrals, documents and registrations are processed and QOF targets are achieved. We have consistently performed well in research at the practice. In 2019 we were the practice with the most samples collected for the BEST3 study.
We are involved in teaching Years 1 & Y3 UEA MBBS students and the training of ST3 doctors. The successful candidate will have the opportunity to become involved in teaching. We are an active participant in Swaffham and Downham Market PCN.

JOB PURPOSE
This post-holder will provide high quality general medical services, holding a list of patients and dealing with a wide range of health needs in a primary care setting, initially on a salaried basis. The successful applicant will show an excellent understanding of the ethos of the practice and will integrate fully into the wider practice team.
We are a well-established practice in the market town of Swaffham, operating from a purpose build conversion of a brick and flint barn, with branch surgeries in the nearby villages of Necton and Narborough. With 7700 patients registered we are forward-thinking and as well as dispensing we carry out research and training (students and ST3s). There will be opportunities to get involved in these areas or develop other practice services. The successful applicant will integrate into our existing team which currently consists of 4 GPs, 1 Paramedic, an Advanced Nurse Practitioner, 3 Practice Nurses, 1 Nurse Associate, 2 highly skilled HCAs, Phlebotomist, Dispensary, Reception and Office staff.
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within a busy primary care setting.
Clinical Responsibilities
Provide a full range of medical services as defined in the core GMS (General Medical Services) contract and additional enhanced services.
In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including:
To keep excellent clinical records including the use of clinical coding.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current practice disease management protocols, developing care plans for health.
Providing counselling and health education.
Referring to other care providers as appropriate.
Prescribing in accordance with the Practice/CCG prescribing formulary (or generically) whenever this is clinically appropriate.
