We have an exciting opportunity for two Consultants in Population Health to join the Director of Population Health working for the third largest Integrated Care System in the Country.

The Cheshire and Merseyside Health Car Partnership serves 2.7 million people across 9 local places.

The Consultants will be working to the Director of Population Health in a new team to improve population health outcomes across Cheshire and Merseyside.

These positions are advertised as NHS Consultant grade or AfC band 9 and are permanent full-time roles.


The postholder(s) will support the Director of Population Health and advise the ICB and strategic partnerships with a focus on prevention and health inequalities.

You will use the best data, intelligence, and evidence to inform service delivery, commissioning, contract management and innovative approaches to change & transformation.


You will be a member of the Population Health team, the Strategic Population Health Board, and cover in the absence of the Director of Population Health at a range of governance forums such as the Integrated Care Board, Health and Care Partnership Board.

You will be accountable to the Director of Population Health.

The postholder will work across the ICB corporate directorates and into the 9 Places across C&


M ICS:

and you are the key interface between the ICB and HCP (known locally as the Health Care Partnership) which oversees the work of the Population Health programme.

The CPH will lead on a portfolio which embraces the main pillars of the population health programme:
All together Fairer - Social Determinants of Health

Supporting Healthy Behaviours Programme

Health Inequalities (Core20Plus5) programme

Strategic Population Health Management and Data into Action programme

Any NHSE delegated prevention programmes which may include section 7a services and other NHSE/I public health responsibilities into the ICB in line with national timescales

The post holders will provide leadership for C&M population health programmes and aligned staff. You will work across C&M and co-ordinate portfolio responsibilities with other Consultants in PH and Local Directors.

You will ensure effective and proactive working relationships with 'Place Directors' and 'Directors of Public Health' and wider ICS colleagues.

The postholder will work with ICB corporate directorates to support wider system wide strategic programme delivery.


The Cheshire and Merseyside Health Care Partnership is the third largest Integrated Care System (ICS) and serves a population of 2.7M people across the local places that comprise of the Cheshire and Merseyside (C&M) sub-region.


The core purposes of the ICS are to:
improve outcomes in population health and healthcare

tackle inequalities in outcomes, experience, and access

enhance productivity and value for money

help the NHS support broader social and economic development.

To take responsibility for the development, implementation, and delivery of policies.

To work closely with corporate and place teams to help create and deliver the 5-year ICB strategy; this will include taking the lead in developing detailed inter-agency and interdisciplinary delivery plans and programmes based on needs assessments which may lead to service specifications.


To deliver on the delegation of any 7a or other public health responsibilities from NHSE/I to C&M ICB in line with national timelines and to provide leadership for the aligned team and their functions.


To support the development of the Integrated Care Partnership (known locally as the HCP), including input into annual work programme and business plan.

To act as an advocate for the principles of population health including reducing health inequalities, a whole-system approach to prevention, addressing the wider determinants of health underpinned by an intelligence led approach of population health management


To work across the partnership to establish key and effective working relationships by working with complex professional, managerial and population groups and other organisations.


To provide senior leadership for Population Health programmes across C&M with a specific focus on the the priorities associated with All Together Fairer (social determinants of health), Healthy Behaviours, Health Inequalities and CORE20PLUS5 programmes.


To collaborate with system leads for Population Health Management and Intelligence (e.g., Combined Intelligence for Population Health Action (CIPHA)), working closely with the ICB's CIO and the Associate Director for Business Intelligence.

Advising on insight and intelligence including prioritisation, risk stratification, data recording, collection systems and annual monitoring to track progress and hold system partners to account for improving outcomes

To support the development of the NHS, public and private sector businesses as Anchor and Social Value organisations to increase local training, employment, procurement and assets and address health
