    Transition to Adult Services Clinical Nurse Specialist - Birmingham, United Kingdom - The Royal Orthopaedic Hospital NHS Foundation Trust

    The post holder will be an autonomous clinical nurse specialist/practitioner working collaboratively, and leading on all components, to provide the effective transition from Children and Young Peoples Services into Adult Services who access the services at the Royal Orthopaedic Hospital NHS Foundation Trust.

    Playing a pivotal role in making a difference, improving patient care and patient experience/care pathways.


    The successful post holder will benefit from the many training opportunities offered by the Trust ,to support both the transition to adult services and their own professional development.


    We are seeking to recruit a marvellously motivated, engaging and energetic clinical nurse specialist to support children, and their families, transition successfully to adult services.


    The focus of this role will be to empower, educate and inform patients and their families through the transitional phase in order that they may continue their care in an adult environment.

    The post holder will provide inspirational leadership and direction to all members of the team and service.

    To have considerable experience and expertise in the care and management of patients during the transition from children services to adult services, and be able to advise and support other members of the hospital and community teams
    To act as an expert resource , giving advice, education/training and support that is appropriately planned to meet the individual's needs empowering them to manage their treatment regimes


    Provide a clinical-led transitional service as a specialised co-ordinator working with a specified group of older children/young adults with complex musculoskeletal, spinal and oncology conditions and any additional complex needs.


    Working withing the wide geographical area that the Trust covers providing expert advice and information through close liaison with a broad range of other disciplines and agencies, including education, social services, voluntary agencies, and primary care and community specialist disability services and other providers.

    To facilitate high quality, individualised care to patients and relatives/carers. Co-ordinates patient care pathways and liaises across organisational boundaries.

    Take responsibility for the ongoing clinical management of this cohort of patients in the clinic and ward/department settings and into the community through liaison with their local health care providers


    To act as an advocate for those families giving expert support and advice based on best practice and close communication with other members of the multi-professional team.


    The postholder will be key at identifying potential development opportunities within the transition pathway and working with the Paediatric leadership team to implement these.

    To project lead and implement change and pathways in partnership with other Health Care Professionals.


    Lead by example, develop yourself and other staff, influence the way care is provided, be open and responsive to the needs of others.

    Work alongside staff to support their development in practice.

    To lead on the running of a support group for transitional patients and their family/carers.


    To actively involve service users in providing feedback of their experiences of the current service and suggestions for improvements through facilitation of support groups and transition workshops also roadshow events.


    To maintain contemporaneous and accurate treatment records, submitting relevant statistics, reports and activity data as required to a high standard.


    To take a lead in developing the quality and availability of transition related information for young people and their family units.

    To ensure the Trust has appropriate policies and processes for transition to adult services are effective and evidence-based care in conjunction with other members of the multi-professional team

    To provide accurate and timely information to inform Trust/National databases as required

    To work within local and national frameworks to implement service changes

    To lead in the provision of high-quality teaching and training in the area of Transition to adult care/services in collaboration with national and local guidance and input from experts by experience

    To firmly embed the early help agenda within the Transitions service by identifying risks or vulnerabilities that could lead to harm or abuse without intervention


    Promote equality, diversity, and rights, and treat others with dignity and respect ensuring services are developed, managed, and delivered to meet the specific needs.

    Promoting health and welling by tackling inequalities.

    Representing and reporting to Trust sub-board committees, such as Children and Young People Meetings internal . Provide written information for patients and their families in the form of educational leaflets and website pages. Adapt communication methods appropriately to ensure the patient voice is heard throughout their care
    BSc Children's Nursing/RN Child/RSCN
    Current professional NMC registration
    Evidence of continuing professional development (CPD) within the last 12 months
    Mentor/assessor training /experience
    Safeguarding Training - Level 3 current

    Possesses or willing to work towards undertaking a master's degree
    Specialist training in Transition to Adult Services, speciality training
    Clinical experience in the care and management of children /young people
    In-depth knowledge of Safeguarding Vulnerable Adults and Child Protection and Mental Capacity Act and Liberty Protection Safeguards
    Organisational skills and ability to lead a team and manage performance
    Have recent clinical experience at Band 6

    Experience in writing and presenting reports
    Research and Audit and benchmarking and service evaluation work
    Counselling skills
    Experience at a Band 7

    Knowledge of Transition to adult services frameworks, such as Ready, Steady, Go and working with these
    Demonstrate a knowledge of duties required for the post , new ways of delivering services
    Well-developed written and verbal communication skills, both verbally and written at various levels with ability to adapt communication as required for children, young people, and families under stress
    Good facilitation, presentation, teaching and assessing skills and support learning
    Ability to move projects forward, to undertake audit activity
    Personal /other qualities
    To identify and employ mechanisms of clinical governance as appropriate, risk management /assessment
    Flexibility to cover service requirements
    Desire to advancement of oneself and others
