- Assist the Social Workers of the Disabled Children's Team in developing support plans for children and young individuals under their care within the Disabled Children's Team.
- Create support strategies for children, youth, and their families in collaboration with both the family and professional networks.
- Occasionally, you may need to participate in and contribute to Team Around the Child meetings and Child in Need Reviews.
- Ensure that objectives are SMART and tailored to the individual child/young person and their family.
- Ensure families comprehend Personal Budgets clearly, including their options for managing them, such as Direct Payments.
- Arranging and determining the cost of support plans as needed.
- Occasionally, meeting with families to assist them in establishing their support plan as necessary or suitable.
- Calculate and organize payment arrangements for support plans.
- Provide guidance to parents regarding the utilization of the pre-paid card and the associated agreement.
- Revise support plans in event of crisis or changes in family circumstances, including recalculating the costs associated with the support plan.
- Appropriate qualifications and/or equivalent experience in healthcare, social services, or education.
- Experience in creating personal budgets and support plans for disabled children and their families.
- Experience in collaborating and communicating with disabled children and their families, along with an understanding of the challenges they encounter in a culturally diverse urban setting characterized by significant deprivation issues.
- Capability to comprehend the requirements of parents and disabled children and facilitate other professionals in aiding these families through a motivational practice and relationship-centered approach.
- Ability to plan and prioritise workloads, work independently and meet deadlines.
- Enhanced DBS registered to the update service.
