    Job summary

    We have recently developed our Learning disability and Neurodiversity (ADHD) Pathway. This is an excited time of change in Blackpool and we would love for you to be a part of the team.

    Job Overview

    To work as part of the CYP Learning Disability and Neuro Team in the ADHD Pathway providing assessment and treatment to children and young people who have complex needs.

  • To support the Team manager in planning, leading and delivering diagnostic support to children and young people who have complex needs.
  • To promote and facilitate access to a range of health services by acting as a key point of contact and working to improve care delivery across the health and social care economy.
  • Contribute to the management and leadership of the team on a daily basis.
  • Main duties of the job

  • To work as an autonomous practitioner managing the pathway in an effective and productive manner.
  • To develop links with primary and secondary health care services in order to maintain effective care pathways, and compliance with local and national strategy.
  • To actively promote service user and family involvement in complex cases.
  • To undertake screening assessments in determining eligibility for the service and ensure any unmet or ineligible needs are forwarded to the relevant agency.
  • To assess and manage complex clinical issues in relation to service-users; can advise and collaborate with carers, professionals and other agencies around complex clinical interventions and outcomes.
  • Can use a range of specialist assessments to identify the needs of service users and to support and mentor other nurses' development in this area.
  • To identify service user related training required by other agencies in order to ensure appropriate person centred care delivery facilitating access to this internally or via links with external NHS colleagues or providers.
  • To contribute to safeguarding investigations which have a health focus.
  • To utilise evidence based practice, policies and procedures and the management of risk.

    Job responsibilities

  • Person Specification

    Qualifications

    Essential

  • Registered professional qualification. , RMN, Social Worker, Occupational Therapists, fully accredited Counsellor
  • Desirable

  • Post registration qualification in child & adolescent qualification in therapeutic approaches with children, young people, parents or carers.
  • Qualification in teaching, assessing, and mentoring of students.
  • Experience

    Essential

  • Relevant post qualifying experience of working within children, young people and families.
  • Significant post- registered qualification experience with children, young people, and families in the area of Neurodiveristy and Learning Disabilities including mental health.
  • Evidence of experience working in a community setting as part of a multidisciplinary team.
  • Evidence of carrying out comprehensive assessments and care planning.
  • Evidence of collaborative working with other services.
  • Evidence of experience and/or training in supervising staff.
  • Personal

  • Evidence of ability to communicate effectively, both written and verbal.
  • Evidence of ability to work as an effective member of a team.
