    This is an exciting time to join our team as the profession of pharmacy further evolves its patient-focussed skillset towards the registration of pharmacist prescribers from day one of qualification, and the profession steps up to incorporate prescribing as a regular part of clinical interventions across all patient-facing sectors.

    Main duties of the job

    To provide leadership and clinical mentorship in the development of the Health Board's primary care pharmacy services through the support, improvement and advancement of the delivery of all pharmacy practice-based roles.

    Be accountable for the development and improvement of patient-facing primary care pharmacy services, leading on the evolution of the current service delivery, focussing on prudent and value based models for delivery.

    To optimise patient outcomes by ensuring that medication is prescribed in line with the current guidelines.

    Demonstrate the highest level of clinical pharmacy input for patient management through your own practice and mentorship of others to the primary care multi-disciplinary team, driving the awareness of pharmacy's patient-focused clinical involvement forward.

    Capture the work that this role undertakes; to demonstrate locally, on a national level, and for publication, how prudent pharmacy working within the primary care team can achieve beneficial outcomes for both individual patients as well as the wider health economy.

    Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:o Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;o Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;o 48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;o Numerous locations providing mental health and learning disabilities services;o Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.

    Qualifications and Knowledge

    Essential

  • Master's degree in Pharmacy or equivalent
  • Post Graduate MSc/Diploma in Clinical Pharmacy or equivalent
  • Registered with the GPhC
  • Independent Prescriber
  • Desirable

  • Recognised postgraduate teaching or Management qualification
  • Recognised postgraduate research qualification
  • Member of the Royal Pharmaceutical Society
  • RPS Faculty member at AS11
  • Accredited foundation designated supervisor
  • Accredited pharmacy diploma tutor
  • Experience

    Essential

  • Substantial postgraduate experience as a pharmacist across multiple disciplines and within the primary care sector
  • Extensive experience of autonomously managing the medication needs of a patient cohort to improve outcomes including the use of prescribing across multiple disease areas
  • Demonstrable safe expansion of competence in prescribing to afford provision of patient care across multiple disease areas
  • Extensive experience of teaching and/or lecturing at postgraduate and undergraduate level
  • Extensive experience in developing, implementing and evaluating teaching programmes.
  • Evidence of clinical audit service improvement, practise research, implementation of procedures and protocols.
  • Knowledge of current strategic plans for pharmacy Services/ development
  • Ability to work with an extensive team harmoniously despite potential differences
  • Good understanding of HR policies and employment legislation to be able to recruit and manage staff
  • Proven change management experience with evaluation
  • Effective management of staff and undertaking staff appraisal
  • Desirable

  • Training & Education experience across healthcare settings
  • Experience of teaching, assessment and appraisal of other healthcare staff/students
  • Published in peer reviewed journals
