Specialist Health Visitor / Clinical Nurse SpecialistAn exciting opportunity has arisen for a full time (1.0 WTE) Clinical Nurse Specialist to join the Community Paediatric Team as part of Ealing Services for Children with Additional Needs within Ealing Community Partners.
This role would also suit an experienced Health Visitor who is looking to transition into the Clinical Nurse Specialist role within in the post.
Are you an experienced Health Visitor, Learning Disability Nurse or Children's Nurse seeking to develop your career in the specialist field of children and young people with complex neuro-developmental disabilities? Do you enjoy and have experience of work with parents who have children and young people with long-term developmental health conditions? This role is an expansion of the existing specialist health visitor roles within the Ealing Child Development Team, looking to transition them into Clinical Nurse Specialists, with a more clinical focus on working with the Community Paediatricians to deliver high quality, patient centred care.
The successful applicant will be based at Carmelita House, working in partnership with the Consultant Paediatricians and other health professionals around the child and family.
The applicant will also have responsibilities in other sites across Ealing and will need to be able to travel around the borough.
We are looking for a nurse with experience of supporting children and young people who have complex developmental needs and can provide high quality care to children affected by their disability in partnership with their family.
This will include carrying out clinical assessments, arranging investigations, making onward referrals, contributing to statutory assessments, delivering sleep training and counselling, liaising with other teams and discussing children at multi-agency and multi-disciplinary meetings.
The post holder will also function as a 'first port of call' for parents looking for support or universal colleagues looking for guidance on complex children.
The successful applicant would need to be able to demonstrate past experience of working with children with special needs, experience of being able to work well in a team, a flexible and innovative approach to healthcare, as well as the ability to work autonomously, and should be committed to the wellbeing of children.
Specific training in clinical assessments will be provided as needed and the post holder will be encouraged to complete nurse-prescribing qualification and then prescribe as needed, particularly within sleep service.

Ealing Community Partners (ECP) is a collaboration of providers that runs Ealing community health services by Ealing CCG.
It is led by West London NHS Trust, and partnered with Central North West London who employs the post holder.
The community paediatric services in Ealing are based in Carmelita House, the home of Ealing Services for Children with Additional Needs (ESCAN).
This is a joint Health / Local Authority venture and is one of the few in the country to host specialist community children's services, children's mental health services, Education and children with disabilities social work team within one building and one managerial entity.
Clinics are also run at other locations across Ealing, and in specialist schools and nurseries.Ealing is a vibrant residential suburb of West London and is the 4th largest borough in North West London with a population of approximately 340,000 people, of which 67,000 are children aged 0-15.
You will receive supervision and appropriate training to keep up to date, to enable you to fulfil your role.The successful candidate will be expected to work full-time Mon-Fri, 9-5, but job share/part-time posts can be considered. Other flexible working arrangements may be possible by discussion with the clinical lead.
This is not an exhaustive list as the role may change further over time according to the needs of the local population and the post-holder's career development.
To provide a contact point for the EaIing Community Paediatric Team for families/carers, other professionals within ESCAN and other agencies, to act as a resource for families to ensure they are aware of appropriate therapies, educational, social, financial, and leisure services to enhance the CYP development and quality of life for their child.
To carry out comprehensive health needs assessments of children and young people with complex needs within different aspects of the overall Child Development Service, for example: carrying out developmental assessments, taking detailed neurodevelopmental histories, carrying out school based observations of social interaction/behaviour.
To contribute/provide reports as required for the Education and Health Care Plan process.Attend relevant meetings, as required, including safeguarding and child protection meetings, team business meetings and specialist meetings eg. Down syndrome pathway, Autism pathway, as needed.
Co-ordinate care for CYP with life limiting conditions and their families/carers, as indicated by consultant or Service Lead, whilst the network around the CYP is being set up
Qualifications and Training

Substantial post registration community experience
Experience in key working and care coordinating
Experience of liaising/negotiating with other professionals and inter-agency collaborative working
Understanding of current health legislation and priorities
Child protection experience
Working knowledge of special educational needs and SEND reforms
Adaptable and flexible approach to work
Report writing skills
Have valid NMC pin
Hold a full, valid, UK driving licence and have access to a car for business purposes.
