Description
NurtureCare is commissioned to support both complex children and young adults with additional health needs within the home environment, we have exciting opportunities for reliable, passionate and dedicated Children and Young Adults Registered Community Nurses across Nottinghamshire, Derbyshire and Leicestershire.
To provide a high standard of nursing care to children and young people with Complex health needs within community.
To asses, provide and review nursing care, advice and information to clients, families and carers within the home environment or alternative care setting.
Aim to wherever possible maintain clients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life.
To work under the direction of the Community MatronWithin the child/young adult's home

Be responsible for the assessment of children and young people's needs. Be involved in the planning, development, implementation, and evaluation of programmes of care.
Continuously assess children and refer to the clinical lead if treatment or care needs to be substantially reviewed.
Provide expert nursing care, advice and support to children and young people with long term and/or complex medical conditions.
This will include chest and limb physiotherapy, oral and nasopharyngeal suctioning, administration of medicines, ventilatory support, tracheostomy care, nutritional care and seizure management, and palliative care.
Facilitate care at home, in other community settings and in hospital. This will include education to support nursing care at home, school, and other community settings.Provide on-going support for children and their families about all aspects of their illness and treatment and provide health education to the child or young person's family to promote a healthy lifestyle.
To educate the multi-agency team to maintain high standards of knowledge and skill for this client group, to enable the child or young person to be nursed at home, for example teachers, carers and the voluntary sector.
Utilise expert skills in assessing a child's condition, including caring for children with complex disabilities, and the equipment needed for the child.
Administer medication to the child/young person according to prescription in the absence of the primary carer, recording all medication administered on a parental permission form.
Facilitate early discharge from hospital and enable delivery of care in the child's own environment.hospital and ward staff, social services, health visitors and specialist nurse regarding the child's needs.
Work as the child's/families advocate, ensuring they are involved in the planning and developing their own plan of care.
Identify and source when required, equipment required to support the treatment and care of the child or young person in the home.
Continually work as a lone worker, not directly supervised in the community, providing care up to a 12-hour period.
