This is an exciting opportunity for a B6 nurse to join the Lewisham Low Intensity Support Team (LIST).


Band 6 Mental Health Practitioners will be based within a Lewisham wide mental health team which provides low intensity integrated health and social care services.

This service includes the Lewisham Clozapine Clinic based at Ladywell out-patients Department in Lewisham Hospital.


Working as part of a multi-disciplinary team alongside colleagues from a range of professional backgrounds, they will coordinate the development of sustainable recovery-focused care plans.

They will provide practical support and carry out planned interventions to address assessed needs and to overcome barriers to access and social inclusion.

Using a strengths-based approach they will support service-users to develop their own solutions, minimising long-term dependence on services through self-directed support.


  • To manage a caseload of mental health serviceusers with complex needs as lead professional. These service users have been stepped down from generic CMHT's and are seen every 3 months, with mínimal input. However, caseloads are generally larger than CMHT's.
  • To use evidencebased and clientcentred principles to assess needs and to plan, implement and evaluate health and social care interventions when these are required.
  • To facilitate flow through the service and timely stepup to other community services in the event of relapse. To assist in the discharge of service users to primary care as indicated.
  • To provide leadership for junior staff through supervision and appraisal.
  • To mentor and supervise students on practice placements
  • To carry out MDT activities e.g. coordinating meetings and undertaking duty responsibilities
  • To provide professionspecific interventions for the wider team as required and in line with NICE guidelines
  • To participate in team training and mandatory training and ongoing professional development

  • CMHT Team Leader, team administrator and clinical team members.
  • Neighbourhood Service Manager and PCMHT colleagues
  • Inpatient service, Crisis Resolution and Home Treatment Team and Psychiatric Liaison Team.


The Lewisham Low Intensity Support Team is an integrated service providing support in primary care for people experiencing complex mental health problems.

The service consists of a multidisciplinary team of mental health practitioners including psychiatrists, nurses, social workers, employment specialists and support workers who work in a recovery focused way, using a strengths-based approach based on individual needs rather than diagnosis.

Key Responsibilities:


1)

Engagement:

Post holder will be responsible for a large caseload of low intensity service users who will require mínimal input and should be seen every 3 months.

However, those service users may require injectable medication and the post holder should ensure that this is dispensed when required.


2)

Support:

To provide ongoing care coordination of a designated caseload of service users. To work collaboratively with service-users and carers using a recovery approach.

To provide information to help clients make an informed choice about opportunities and services available to them and to then assist clients to obtain the required services, support and entitlement.


3)

Interventions:

To provide and facilitate health and social care interventions tailored to individual service-user needs.

To ensure all interventions are appropriate and that care is sensitive to the needs of black and minority ethnic service users.

To provide psychoeducation to services-users if required. To have a client-centred approach with a focus on recovery and social inclusion. To assist in the administration of injectable medication, and adherence for those attending the Clozapine Clinic.


4)

Clozapine Clinic:

To participate in the running of Lewisham Clozapine Clinic on Tuesdays and Wednesdays. Training will be provided on how to use Point of care equipment.

Families andCarers:

To involve families and carers in treatment plans where appropriate. This may require direct fam
