Our Public Health Nurse teams are innovative, enthusiastic and motivated to deliver the best healthy outcomes for children and families within Devon. Excellent peer-based support and supervision will be given while you develop your knowledge, skills and competence.
You should have evidence of recent professional development and an understanding of Community Practice and Public Health Issues. If you have the drive and passion to influence healthy outcomes for communities and families and in doing so help make a difference to the lives of children and families then we would like to hear from you.
Main duties of the job
Participate in the assessment of health needs and formulation of care plans for children, young people, their families and communities. This includes the implementation of programmes of care, offering support and advice to parents and families in the home, clinic, school or community setting ie: parenting skills, public health, group work and community clinics.
In collaboration with the public health nursing team, identify individuals and groups within the community where collaborative working and advice would forward the interests of public health and promote healthy living, in particular seeking to identify disadvantaged groups and address health inequity.
Offers advice and support to children, young people and adults in accordance with evidence based practice. To actively take responsibility for appropriately identifying, accessing and participating in supervision, according to individual need Child Protection, Restorative, and Clinical.
Support the delivery of key performance indicators by working collectively across wider geographical areas.
