We are delighted to offer this exciting opportunity for two appropriately experienced and registered health or social care professionals to work as part of the MND Care Coordinator Service that works across North Wales.
The MND Care Coordinator role is responsible for co-ordination of care and support to people living with MND in North Wales.
The posts are based in the West (covering Mon, Gwynedd, Conwy and some of Denbighshire) or the East ( covering Denbighshire, Flintshire and Wrexham) The post holders will be normally expected to work Monday - Wednesday or Wednesday to Friday each week, during the hours of - 5pm.
Applications are welcomed from professionals registered with any health or social care professional body, such as nursing, social work or the allied health professions.
If you have the relevant qualifications, experience, knowledge and skills, together with a strong desire to improve care for people with MND and their families, then we would love to hear from you.
The post holder will be expected to:
Deliver local support to patients diagnosed with MND in North Wales as a key point of contact.
Track patients along their disease pathway from diagnosis to end of life.
Coordinate the local multidisciplinary care of patients with MND across North Wales in primary and secondary and third sector care ensuring patients can access high quality care that adheres to the best practice guidance.
Liaise with the Consultant Neurologists and the MND Care Centre in the Walton Centre NHS Foundation Trust at Liverpool for specialist input of MND care.
To be a single point of contact for patients with MND, their carers and professionals as a resource for information advice and signposting.
