    The Release and Transfer team plays an integral part in the support provided in our patient journey. As a Release and Transfer nurse you will use your expert clinical skills and demonstrate judgement in assessing and managing wide ranging and complex patient problems. You will provide care and planning for adult male prisoners from the point of reception until the day of release or transfer to another establishment. You will be expected to work collaboratively with the patient and any other agencies to support a safe transition back into the community/ other establishment. You will conduct a holistic patient assessment and manage the assessment of health needs, planning, delivery and evaluation of care for the prison population, including clinical risk assessment.

    Main duties of the job

    We are looking for a first level Registered Nurse with current registration with NMC. You will have experience of implementing local and national policies NICE guidance, developing protocols/care pathways and monitoring effectiveness. You'll have a non-judgmental and compassionate approach will enable you to succeed in this role, as well as a proactive approach to problem solving.
