Pharmacy technicians play animportant role, complementing clinical pharmacists and other members of the PCNmulti-disciplinary team.
The post holder willhelp patients to get the best from their medicines, operating within clearprotocols and guidance. They will be responsible for encouraging thedevelopment of better understanding of the principles of medicines optimisationthroughout the practice and PCN teams and promoting good practice in line withtherapeutic developments.

Main duties of the job
The post holder will contribute to improving prescribing practice, patient care This includes improving repeat prescribing processes in General Practices within the PCN, promoting digitalisation, reducing medicines waste and minimising clinical risk associated with medicines taking, support medication reviews and medicine recognition.

Primary Duties andAreas of Responsibilities
All PharmacyTechnicians must work within their competencies.
Clinical:
Undertakingpatient facing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients.
Carryingout medicines optimisation tasks including effective medicine administration( checking inhaler technique), supporting medication reviews and medicinesreconciliation. Where required, utilise consultation skills to work inpartnership with patients to ensure they use their medicines effectively.
supportingmedication reviews and medicines reconciliation for new care home patients andsynchronising medicines for patient transfers between care settings, linkingwith local community pharmacists.
Supportingthe Clinical Pharmacists to provide medication reviews/ Structured MedicationReviews (SMR) organising necessary monitoring tests prior to SMR
Providingspecialist expertise, where competent, to address both the public health andsocial care needs of patients, including lifestyle advice, service information,and help in tackling local health inequalities.
Supportinginitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing.
Torespond to medication queries for patients and staff in a professional andtimely manner
Technical and Administrative:
Working with the Practice multi-disciplinaryteam to ensure efficient medicines optimisation, including implementingefficient ordering and reducing wastage.
Providingtraining and support on the legal, safe and secure handling of medicines,
Developingrelationships with other pharmacy technicians, pharmacists and members of themulti-disciplinary team to support integration of the pharmacy team acrosshealth and social care including primary care, community pharmacy, secondarycare and mental health.
Workingwith practice clerks and reception teams in sorting and streaming generalprescription requests, so as to allow GPs and clinical pharmacists to reviewthe more clinically complex requests.
Therole will also require pharmacy technicians to support the implementation ofnational prescribing policies and guidance within GP practices, care homes andother primary care settings, supporting quality improvement measures andcontributing to the Quality and Outcomes Framework and enhanced services. Inaddition, pharmacy technicians will assist in the delivery of medicinesoptimisation incentive schemes ( medicines switches) and patient safetyaudits.
Pharmacytechnicians will provide leadership for medicines management systems,supporting practices with a range of services to get the best value frommedicines by encouraging and implementing Electronic Prescriptions, safe repeatprescribing systems, and timely monitoring and management of high-riskmedicines).
Toadvise on the sourcing and safe management of medicines as appropriate.
Toassist in the submission of claims for personally administered drugs asrequired.

The post-holder will participate in any training programmerequired by practice, including the approved Primary Care Pharmacy EducationalPathway (PCPEP).
Participation in an annual individual performance review,including taking responsibility for maintaining a record of own personal and orprofessional development.
