Main duties of the job
PALMS (positive behaviour, autism, learningdisability and mental health service) works across Hertfordshire providing aspecialist multidisciplinary approach to children and young people aged 0-18who have a global learning disability and/or autistic spectrum disorder andtheir families. PALMS clinicians comprise a virtual team delivering aconsistent service in a variety of community settings.
PALMS is amulti-disciplinary team who provide individually tailored support to helpmanage a number of concerns experienced at home. The service is available forchildren and young people up to the age of 18.

Job descriptionJob responsibilities
To provide acomprehensive screening and triage service to CYP and their referrers resultingin safe and appropriate clinical decisions and appropriate intervention.
Timely and effectivetriaging of all referrals adhering to the service specification, pathways andprotocols to support clinical decisions.
To be the firstclinical point of contact for referrers and the families who access, or enquireabout accessing, PALMS following agreed protocols and guidelines and utilisingeffective communication skills. The calls can include assessing risk and thepriority of the referral.
Ensure thatsufficient, accurate and appropriate information is gathered in order to directthe referral to the most appropriate pathway.
Use professionaljudgement and skills to assess level of need and urgency, as well as obtainingfurther information as required, determining appropriate outcome includingmonitoring and recording expected outcomes. Agreed pathways and protocols willbe used to support clinical decisions.
To be a point ofcontact for team members to discuss clinical queries that cannot wait untilsupervision or peer reflection.
To undertake priorityassessments, related to difficulties within the PALMS remit, to determine theneeds of the CYP, including whether it is safe for them to return to theirhome.
