    Job summary

    Anexciting opportunity has arisen for another experienced, enthusiastic and committed CLINICAL PHARMACIST to join the practice pharmacy team at Riverside Health Partnership. Experience is essential, and our ambition is to find the right candidate with the necessary skills and expertise.

    This position is part time (hours negotiable with applicant) withthe possibility of flexibility, working across Monday to Friday.

    At Riverside, we have a very experienced and proactive pharmacy team, including Clinical Pharmacists, Pharmacy Technicians, experienced medicines management staff, and dispensing assistants. We believe the addition of another part-time pharmacist will strengthen the team and develop services even further for our patients.

    The salary is Band 7 - 8A depending on qualifications and experience

    We also offer 27 day weeks paid annual leave and the opportunity to join NHS pension scheme.

    Main duties of the job

    The successful candidate will be passionate and enthusiastic, well organised,highly motivated and skilled in their role.

    This role will be to lead a well-established team, working closely with your colleagues across the practice, initiating smarter ways of working to maximise the time and reach the expectations of our patients. The ethos of the practice and PCN is ensure excellent support for staff and patients and on-going development as part of the wider team.

    Workingwith practice colleagues, we aim to enhance service delivery for our patients, alongside ongoing opportunities and for Riverside Health Partnership within the Retford andVillages Primary Care Network.

    Professional registration is essential for this role and we would be delighted to have the addition of a prescriber with management experience to fulfil this vital role in the practice.

    Retford& Villages PCN comprises of 5 General Practices: Riverside Health Centre,Kingfisher Family Practice, Crown House Surgery, Tuxford Medical Centre andNorth Leverton Surgery. With a population of over 54,000 people, it is the largestof the three PCNs in Bassetlaw.

    Riverside Health Partnership serves the North Nottinghamshire market town of Retford and its surrounding villages. We are an established, well-respected, progressive and popular practice providing services from our sites in Retford, Misterton, Harworth and Gringley-on-the-hill.

    The partnership has 7 GP partners (including 1 Managing GPpartner), 7 Salaried GPs, 5 Advanced Nurse Practitioners (including Clinical Operational Manager and Nurse Team Lead), Practice Manager and a structure ofmiddle and junior managers and team leads.

    Job responsibilities

    Working with the whole medicines management and pharmacy team toensure that patients receive a high level of care and service delivery.

    To work as part of the primary care team in the practice supporting the staff to help achieve efficiencies and effectiveness.

    To support with leading improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care within the practice. To improve repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.

    work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

    responsibility for the care management of patients with chronic diseases and undertake clinical structured medication reviews to proactively manage people with complex polypharmacy.

    ensure patients have appropriate monitoring tests in place when required.

    4.Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients and to help in tackling inequalities.

    5.Provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.

    6.Support patients to take their medications to get the best from them, reduce waste and promote self-care, through structured medication reviews.

    7.Support the integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.

    12.Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components ( advice on treatment pathways and patient information leaflets)

    Essential

  • Excellent advanced theoretical and practical knowledge of PCNs, General Practice and evidence based medicine
  • Excellent clinical pharmacy knowledge including terminology
  • Excellent knowledge and understanding of pharmacy law and ethics and current legislation
  • Ability to analyse, interpret and evaluate specialist medicines related information or data.
  • Ability to communicate sensitive and complex specialist medicines related information and advice to patients & carers, medical teams and other healthcare professionals. Types of sensitive and specialist medicines and pharmaceutical information include: choice of therapy (including formulary issues), choice of formulation and route of administration, side effects, adverse drug reactions, use of drugs where its action and effects may be influenced by patient disease state, biochemical parameters, liver and kidney function, diet, gender, ethnicity, pregnancy, breast feeding, age related problems. This could be in sensitive situations including cancer treatment, terminal illness and the effect of drug administration or prescribing errors or other drug misadventures. Discussions may be on a one-to-one basis with patient, doctor or other healthcare professional, or by telephone, e-mail or letter; or a one to many on medical ward rounds.
  • Ability to analyse, interpret and evaluate specialist medicines related information or data, using clinical and professional judgement. This includes interpreting reports from any pharmacy computer system about drug usage and expenditure, clinical intervention reports, data from clinical trials and published papers, and changes in medicines legislation and policy (local and national). This information could inform decisions about an individual patients drug treatment ( data from the pathology system) or protocol & guideline development
  • Self-motivated and able to work independently and autonomously as required, without the need for close supervision or support.
  • Support the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance.
  • Computer literate with an ability to use the required GP clinical systems (desirable) and Microsoft office packages (essential)

  • Qualifications

    Essential

  • Masters degree in pharmacy or equivalent
  • Postgraduate clinical qualification and/or significant relevant clinical experience to demonstrate similar level of knowledge
  • Registered with the General Pharmaceutical Council
  • Minimum of 2 years experience as a pharmacist, in acute hospital, community pharmacy or community health services, demonstrated within a practice portfolio.
  • Experience of working with the public.
  • Experience of being responsible and accountable for own professional actions, working independently within professional and organisational boundaries and being responsible for the actions of others within their area of responsibility
  • Experience in planning, developing and delivering pharmacy services
  • Desirable

  • Completed Centre for Pharmacy Postgraduate Education (CPPE) Clinical Pharmacist 18 month training pathway.
