    Job summary

    In line with the philosophy of the clinical area, the post holder will be responsible for the assessment, planning, implementation and evaluation of care for the service user, in conjunction with other staff.

    The post holder will provide a key leadership role providing direction to the team, acting as a positive role model and contributing to the professional development of junior staff and consistently demonstrating Trust Values.

    The post holder will work in close collaboration with other professionals, including the wider Multi-Disciplinary team (nursing, psychiatry, psychology, occupational therapy and speech and language therapy) in assessing service user's needs, devising care plans and evaluating outcomes that promote service user empowerment through the process.

    Work as part of the Trust shift rota across a 24 hour shift pattern including Long day shifts and Nights.

    Main duties of the job

  • Provide leadership for the team, developing working partnerships with inpatient services and primary, secondary and tertiary health care.
  • Provide management supervision and appraisal for junior staff, in accordance with the HPFT supervision policy.
  • Function as a charge nurse to people with learning disabilities and associated mental health needs, taking responsibility for formulation of appropriate care plans.
  • Act as a clinical expert in their service area. This means keeping clinically up to date, being able to assess service users thoroughly, plan care rigorously and audit and evaluate nursing and professional specific interventions, based on evidence and locally agreed protocols and policies.
  • Act in a consultative role to external agencies in relation to clinical interventions, risk assessments and management plans.

    Job responsibilities

  • Co-ordinate all co-workers assigned to their service user group and ensure they are fully conversant with all aspects regarding the service users team and the MDT.
  • Identify the individual care needs of service users. Plan, implement and evaluate all care programmes, in conjunction with the community nursing team and the MDT.
  • Provide effective clinical support and line management of nurses, and health care assistants within the service.
  • Provide nursing care in a person-centred way that promotes and facilitates the positive health and well-being of people with learning disabilities.
  • Maintain close liaison with all departments and the staff involved in service users care and report on progress and effectiveness to the MDT.
  • Lead and/or participate in assessments within community and inpatient settings and services and prepare reports on service users, as required.
  • Be aware of all policy document and legal requirements pertinent to the post and ensure that these are adhered to.
  • Demonstrate and serve as an example for all clinical activities to appropriate grades of staff.
  • Be fully conversant and ensure compliance with their relevant professional bodys codes of conduct and practice, relevant legislation and policies and procedures.
  • The post holder is required to maintain current registration with their relevant professional body.

    QUALIFICATIONS/EDUCATION/TRAINING

  • Relevant Professional Registration with Nursing and Midwifery Council (NMC).

  • PREVIOUS EXPERIENCE

    Essential

  • Significant post graduation experience within Learning Disability Service
  • Knowledge of common physical, mental and emotional health needs experienced by people with a learning disability.
  • Experience of working with people with learning disabilities or complex health needs.
  • Knowledge of legislation and policy relating to learning disabilities.
  • Skills and Knowledge

    Essential

  • Experience of maintaining high standards of recordkeeping consistent with legislation, policies and procedures.
  • Knowledge of common physical, mental and emotional health needs experienced by people with a learning disability.
  • Physically fit and able to successfully complete a prevention and management of aggression training course, and annual refresher training.
  • Contribute to a culture that promotes equality and diversity, including recognising and reporting behaviour that undermines this.
