    AKTON Recruitment is working closely with the local authorities and we are looking for a Support Planner in the Islington area to join the Children's Services.

    PRIMARY JOB FUNCTION
    You will be part of the Disabled Children's Service, support planning for disabled children
    and young people who have a Social Worker supported by the Disabled Children's Social
    Work Team.

    This also include supporting social workers to further develop their practice skills regarding support planning for disabled children through modelling good practice.

    Your role will be and devise to assist and support the Disabled Children Team social workers to develop support plans in line with needs, goals and outcomes identified in C&F single assessments and CIN Reviews using personal budgets.

    You will also work closely with members of the Direct Payments Team to ensure families are supported and advised to access employment advice and advice regarding direct payment arrangements and pre-paid cards DUTIES AND RESPONSIBILITIES

    • Support the Disabled Children's Team Social Workers to support plan for children
    and young people who are held within the Disabled Children's Team

    • Develop support plans for children and young people and their families, in
    partnership with the family and the professional network

    • May be required at times to attend and contribute to Team Around the Child
    meetings and Child in Need Reviews

    • Ensure that goals are SMART and personalised to the child/young person and their
    family

    • Ensure that families have a clear understanding of Personal Budgets and their
    options for managing this, including Direct Payments EDUCATION and EXPERIENCE

    E1 Relevant qualification and / or equivalent experience in health, social care or education E2 Experience of working and communicating with disabled children and their families and an understanding of the barriers they face in a culturally diverse urban environment where there are significant deprivation issues E3 Experience of devising personal budget and support plans for disabled children and their families.

    KNOWLEDGE, SKILLS and ABILITY
    E5 Ability to identify safeguarding concerns and follow appropriate processes to manage / reduce risk against children.

    E6 Knowledge of relevant legislation, national standards, policy and practice and current issues in the provision of services for severely disabled children and their families E7 Experience of working partnership with short break providers and care agencies.

    E8 Ability to understand the needs of parents and disabled children, and be able to help other professionals to support these families using a motivational practice and relationship based approach.
