    Job summary

    An exciting opportunity hasarisen for a proactive, patient-centred Occupational Therapist to join a smallteam of OTs within East Lindsey Primary Care Network.

    We are looking for 2 Part-time OTs. 1 x 24 hours to cover Woodhall Spa and New Coningsby and 1 x 15 hours to cover Caistor.

    Occupational Therapy is at thecore of supporting patients to live well, maintain function and recover fromill health and our Occupational Therapy team will be an integral part ofimproving the wellbeing of our patients in the NHS Long term plan and thedeveloping Lincolnshire Frailty Strategy.

    The successful candidate willbe required to utilise their in-depth knowledge, clinical skills and experienceat a senior level, in the management of patients Occupational Therapyconditions within the remit of falls and frailty. We are focused on earlyintervention with a goal to support patients proactively, to avoid falls andprevent or slow down frailty progression. Patients will be seen in a variety ofsettings including GP surgeries, care homes and domiciliary settings.

    The post holder will work collaboratively with the PCN and surgery based multi-disciplinaryteams (MDT) to meet the needs of patients and carers, support the delivery ofpolicy and procedures, and provide Occupational Therapy education as required.

    formulate and apply relevant occupational analysis to theindividual, their activities, and their environment

    Ensure that all patients that have unplanned contact withurgent care settings have their needs addressed where appropriate.

    Ability to motivate patients to engage in self-management oftheir health conditions

    help patients to adapt to and manage their physical andmental health long-term conditions, through the teaching of coping strategies

    Ability to deliver Occupational Therapy intervention on bothan individual and group basis to include presenting at pop up events hosted bythe PCN

    Have the knowledge of, sign post and refer on to otherprofessionals and agencies where appropriate, to ensure holistic patientintervention.

    Possess good communication skills in both verbal and writtenforms to allow effective MDT working and communication with patients and theircarers, considering alternative modes of communication where appropriate.

    Work collaboratively with social services and other communitycolleagues and to enhance the patientexperience.

    develop,implement and evaluate a seamless occupational therapy support service acrossthe PCN, working with community and secondary care where appropriate, and aimedat continuously improving standards of patient care and wider MDT working

    maintainan up to date knowledge of self-management advice, specialist equipment,environmental adaptation, technology and legislative responsibilities which maysupport increased function and/or management (for onward referral / request tosecondary care)

    advise patients, and their families or carers, on specialistequipment, adaptations and organisations that can help with daily activities

    Attend practice/multi-disciplinary/community meetings whererequired.

    Contribute to the development of the Occupational TherapyService within East Lindsey Primary Care Network.

    develop and design patient and carer information, includingadvice to support management of conditions where needed

    provide education and specialist expertise to PCN staff,raising awareness of good practice occupational therapy techniques

    ensure delivery of best practice in clinical practice,caseload management, education, research, and audit, to achieve corporate PCNand local population objectives as directed

    Apply policies and procedures in line with the OccupationalTherapy Profession and Practices along with those of ELPCN.

    Support the general practice team to ensure compliance withCQC standards.

    Support practice level QOF activity where required.

    Attend training and supervision sessions as required andcontribute to in house cpd sessions/shared learning.

    Qualifications

    Essential

  • Qualifications:
  • BSc in Occupational Therapy under a training programme approved by the College of OTs.
  • Health & Care Professions Council (HCPC) registration.
  • Able to operate at an advanced level of clinical practice.
  • Knowledge:
  • Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
  • Analytical skills:
  • Ability to evidence a sound understanding of the NHS principles and values.
  • Independent thinker with good judgement, problem-solving and analytical skills.
  • Ability to write comprehensive clinical notes, implement and evaluate care plans.
  • Experience of assessment and treatment delivery in long term conditions and active management.
