- To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the practice manager.

Main duties of the job
To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
About us
We are a rural practice basedacross 2 sites in the villages of Kirton Lindsey and Scotter and it will benecessary to work across both sites as required. We currently haveapproximately 10500 patients, which is ever increasing. We have a CQC rating ofGOOD. We use SystmOne (clinical system) and AccuRx (Online).
We have a multidisciplinary, clinical team currently made up ofDoctors, an ANP, Practice Nurses & HCAs as well as a wider team consistingof Pharmacists, Musculoskeletal Practitioners, Physios, Care Co-ordinators andNurse Associate through the Primary Care Network. Our doctors are currentlyinvolved in GP training, CCG and PCN working. We are also a training practicefor GP Registrars, HYMS Students and F2s.
