Due to impending retirement, Swallownest Health Centre are looking to appoint a highly motivated, enthusiastic, dedicated, passionate and caring Lead Practice Nurse to provide cover 30 hours plus per week. The successful candidate will have experience in chronic disease management. Experience of Primary Care is essential.
Practice Nurses within our practice are an important part of the clinical team. The successful candidate will lead our dedicated Nursing team and support GPs and the Nursing team in the care of practice patients to include treatment, preventative care, screening and patient education.
It is essential that you have a flexible approach to your working hours in order to fit with the demand/space available within the practice, which is limited.

Main duties of the job
Tobe responsible for the management of the practice nursing team. Working as part of the practice multidisciplinary team, thepost holder will ensure nursing services are delivered effectively and efficiently. The lead practice nurse will be responsible for anumber of clinical areas such as infection prevention and control, health promotion, chronic diseasemanagement, health prevention, as well assupporting the practice management team in thereviewing and delivery of Health & Safety requirements along with clinical policy and procedure.
Provide nursing treatments along with general and specific health screenings, minor ailments and chronic disease management to patients in participation with general practitioners or independently to agreed protocols.
Undertake the collection, storage and dispatch of pathological specimens including intravenous blood samples, swabs, smears etc.
Use of a computer to manage daily workload; email, messaging, basic word and excel however clinical system training will be provided.

Jobsummary:
The post holder is responsible for the delivery of generalpractice nursing to the whole practice population. The focus of therole includes the delivery of evidence-based practice for patients presentingwith chronic disease management, and the provision of preventative health careto the practice population. As an autonomous practitioner the nurse isresponsible for the care delivered, demonstrating critical thinking and skillsin clinical decision-making in the management of patients. They will workcollaboratively within the general practice team to meet the needs of patients,support the delivery of policy and procedures, and provide nurse leadership asrequired.
Mainduties and Responsibilities:
To assess, plan,implement and evaluate clinical nursing care to provide services in;
Treatment room.
Cervical smears.
All aspects of Ear care.
Health promotion.
Deliver care in chronic disease management.
Flexibility to take on extra hours / changeshifts during annual leave or during
periods ofabsence as required or event of pandemic.
Maintains high standards of infection controlat all times thus protecting patients, self and colleagues.
Actively engages patients and carers in themanagement of their health, promoting health and wellbeing.
Refers patients to other members of thepractice team in line with agreed protocols.
Maintains accurate records of all clinicalactivity in line with practice protocols and standards.
Totriage patients over the phone to the appropriate member of clinical staff withthe appropriate skills to meet a patients stated needs.
Contributes to clinical governance activitieswithin the team including audit and evaluation and seeks to underpin practicewith current evidence of effectiveness.
Supports members of the practice team tomaintain health, safety and security.
Contributes to teaching and offers support tofellow colleagues within the practice team.
Prioritise, organiseand manage own workload in a manner that maintains and promotes quality.
Uses appraisal and supervision to identify owndevelopment needs and maintains and develops competence in line with agreedrequirements.
Stock control / stock rooms / keeps up with healthpromotion displays / activity and other administration duties as deemedappropriate.
