    Hillingdon Community Diabetes service are looking for a motivated band 5/ 6 dietitian to work within the Diabetes MDT. Working closely with the Band 7 diabetes specialist dietitian The post holder will be supporting patients living with diabetes across the borough of Hillingdon. This will include working both in a clinical and domiciliary setting as well as providing training to GPs and Practice nurses in Diabetes Care.

    Main duties of the job
    • To develop and provide clinics, domiciliary service (if applicable) and telephone/ remote clinics (if applicable) for patients with diabetes.
    • To work closely with a multidisciplinary team caring for diabetes in the community.
    • To act as a source of expertise and develop and provide specialist training on diabetes including weight management, lipid modification and carbohydrate counting.
    • To develop and provide education and training to student dietitians and community groups
    • To carry out audit and research in the field of diabetes and nutritional management.

Job description Job responsibilities
    • Clinical Practice Support the management of insulin and/or change insulin regimens for patients with poor diabetes control in any setting including in clinics, at home and in GP surgeries + other venues as appropriate. Monitor patients ongoing glycaemic control and titrate insulin / oral medication appropriately in the community setting, patients homes or by giving telephone advice. Work to the Dietetic Competency Framework for Dietitians Working in the Area of Diabetes. Support, advise and recommend treatment regimens for diabetes and other co-morbidities including hypertension and lipid management in accordance with national guidelines, for example BHS & NICE guidance. Assess & triage patient referrals accordingly. Prioritise and action as required.
    • Communicate across multi-disciplinary teams as appropriate. Provide telephone support to patients, GP practices and other healthcare professionals accordingly. Undertake mandatory training & clinical supervision as per trust policy. Prioritise & deliver diabetes clinics to improve diabetic control and reducediabetes complications. Maintain necessary level of equipment & resources to be able to carry out necessary clinical work within general practice setting, for example blood glucose meters, demonstration insulin pens, appropriate patient information literature etc.
    • There is potential risk for needle stick injuries etc thereforeawareness and compliance with trust infection control policies essential. Hepatitis B immunisation and status must be up to date.Health Promotion Recognise patients readiness to change by supporting their attempts to change unhealthy behaviour Provide healthy eating advice to individuals or to groups Provide smoking cessation advice in opportunistic consultations Provide individual advice to patients about increasing activity or exercise Offer advice on recommended safe limits for alcohol consumption Provide an opportunity to discuss the use of recreational drugs Develop and maintain the Health Promotion material Refer to other teams as appropriate Identify gaps in current knowledge base in any HCP and disseminate specialist knowledge in order to improve patient care. Has responsibility for inputting and submitting statistical data as required
    • Provide effective teaching to individual clients and their families.. Develop knowledge base and skills of other staff through formal and informal training programmes organised on a continuum. Contribute to the development of educational materials and the development of practical skills as required Acts as an expert resource on nutrition to Diabetes team. To guide junior members of the team in their development in Diabetes care Plans, organises and implements effective learning for clients and carers Plans, organises and implements effective learning and development opportunities for pre-registration students/junior staff Acts as a facilitator/assessor for pre-registration students/junior staff Mentor pre registration students, participating in the programme design for the community placement Has a range and depth of knowledge of the Trusts policies and procedures Knows how to access a range of resources in order to create a learning environment for student pre- and post-registrationAct as a clinical supervisor, and mentor to HCP colleagues Encourage innovation and help develop new approaches to client care. Participate in the planning, implementation, delivery and audit of structured patient education sessions for both newly diagnosed patients and those with poor glycaemic control. Education also to be cascaded to families and carers as required. Participate in the planning implementation and delivery of multi-disciplinary training for health care workers and professionals e.g. Topical, PitStop or locally derived training programs. Maintain Registration and Revalidation. Maintain accurate, contemporaneous and legible patients records and prepare reports as required keeping accurate records of daily activities. Effective team member Ensure own professional knowledge remains current and is reflective of changes and innovations in practice Inform the appropriate managers of incidents, accidents and drug errors, and requirements and issues influencing the workload and standards of care. Attend Trust mandatory study days. Research and Audit Prescribes and implements evidence based practice at all times Gives sound, evidence-based rationale for decisions when challenged

    • Keeps up to date with, interpret and critically evaluate current literature in dietetic and diabetes practice Communicate research findings to Team by relating them through clients problems. Participate in the appropriate clinical audit of service in line with Trust guidelines and standards. Support and help develop protocols and guidelines within the Trust. Ensure appropriate audit of service is undertaken and data is provided to help inform and improve practice. Participate in local/national meetings/networks as appropriate. Work in partnership with multidisciplinary colleagues working across statutory and voluntary sector. Participate in collaborative clinics/services Promotes effective communication and liaison between primary care and secondary care, optimizing patient satisfaction. Facilitates continuity of care and promotes multidisciplinary care for people with diabetes Participate multi prof. guidelines/policies Partnership working Communication Effective team member Engaging team member Liaises effectively with GP and other key agencies and understands the communication systems within the team and GPs message taking service and others Work in partnership with multidisciplinary colleagues.     Person Specification Qualifications Essential
    • Degree/Post-graduate Diploma
    • Health Professions Council Registration
    • Evidence of post registration education and training including BDA specialist courses in the field of Diabetes
    Desirable
    • Member of BDA DSG Certified XPERT or DESMOND or other accredited diabetes programmes
    • Formal teaching certificate or willing to undertake.
    Experience Essential
    • Post-registration experience in clinical dietetics
    • Extensive experience in the nutritional management of patients diagnosed with diabetes
    • Experience of training/teaching healthcare professionals/students
    • Experience of conducting audits/research and evaluations
    • Experience of working effectively with minimal supervision.
    • Knowledge of developing and implementing policies, guidelines, standards and implementing care pathways. Knowledge of diversity issues and ability to work with people from a wide variety of backgrounds
    • IT Competent (using power point to prepare presentations, dietary analysis packages, databases and undertake literature searches
    • Experience of carb counting and competent in managing insulin titration
