    About the role:

    We have full time (37 hours) and part time (22.2 hours) Adult Social Care Practitioner positions available in our Care Home Review Team.

    Our supportive and engaged team structure includes an Integrated Social Care Manager, a Social Care Lead, Adult Social Care Practitioners and an administration team.

    As an Adult Social Care Practitioner in the Care Home Review Team, you will be primarily focusing on people who live in Care Homes - undertaking assessments and support plans with people who have placed within a care home setting and are due for a review of the care they are receiving to ensure there are no changes which need addressing. The role involves working with people when a care home is closing and addressing quality concerns and safeguarding. This role will involve working with families, carers and other professionals, agencies and organisations across Gloucestershire.

    The ideal candidate will be someone who has experience of working in a health and/or social care setting and who has knowledge of the Care Act and Mental Capacity Act; and the ability to apply it in practice.

    The nature of this post requires traveling around the County to Care homes. You will be predominantly traveling between Dean House in Cinderford and the Cotswolds. One day a week will be based at Shire Hall in Gloucester.

    We work flexibly with a mix of office and home working although during the induction period you will be based at Dean House in Cinderford (for approximately 6 weeks). A full induction programme and training will be available.

  • relevant qualification equivalent to NVQ Level 3 or equivalent Health and Social Care experience
  • training related to Health and Social Care assessments
  • experience of managing a caseload including working with people experiencing a range of physical disabilities, medical, mental health difficulties and learning difficulties
  • evidence of good interpersonal skills, good verbal / written communication
  • good IT skills
