    Job summary

    The post holder will be responsible for assessing and determining eligibility for people with complex health needs whom are eligible for NHS funding for long term care and treatments under The National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, The Mental Health Act. The service also commissions packages of care for clients requiring Transforming Care and Neurological Rehabilitation pathways. The post holder will be responsible to clinically plan and commission the care required, ensuring that the care is safe, effective and good quality. Communication and relationships with individuals and independent providers is a key part of this role as well as abilities to manage risks, problem solve influence and negotiate to ensure appropriate clinical commissioning case management of complex individuals. The post holder will chair Multi-disciplinary meetings in order to make eligibilityrecommendations for NHS Continuing Healthcare and NHS Funded Nursing Care. Complete reviews of commissioned packages ensuring that people are receiving high quality safe and effective care as well as ensuring they still have eligibility for the relevant funding stream. You will also provide expert advice to other professionals commissioned to provide care in relation to NHS continuing Healthcare and NHS

    Main duties of the job

    The post holder will act as Individualised Nurse Assessor for patients whose care is either jointly or solely funded by NHS GM and the care is provided by the independent Identify and case manage clients who may be eligible for NHS funding, chair and coordinate eligibility assessments and Provide expert advice to other professionals commissioned to provide care in relation to NHS Individualised Complete the restitution process for all claims which includes presentation of findings to the NHS GM multi-agency funding panel and the NHS England Independent review Contribute to the quality monitoring of care home providers

    Job responsibilities

    Co-ordinate and manage multi-disciplinary meetings ensuring all appropriate information and evidence is obtained & considered to make decisions regarding eligibility for NHS Continuing Healthcare. Complete concise and accurate reports.

    Communication Understand and communicate complex information, both patient and nonpatient related. Communicate complex and often sensitive and distressing information on a regular basis. Manage complex, sensitive and sometimes contentious situations Nurture key relationships and maintain internally and externally. Contribute to the safeguarding and quality agendas Establish partnerships and networks with partner agencies Maintain an educational and advisory role to others in relation to NHSContinuing Healthcare, Funded Nursing Care and Personal Health budgets.

    INFORMATION RESOURCES, ANALYSIS AND DECISION MAKING Responsible for producing assessments and reports using appropriateinformation technology. Maintain up-to-date, accurate records, inputting case management into the electronic patient system. Collect, collate, evaluate and report clinical information to inform eligibility and commissioning decisions. Provide information to assist in the production of statistical returns. Reporting all incidents, near misses and hazards in line with NHS GMreporting arrangements/system.

    PLANNING & ORGANISATION Work autonomously, prioritising and managing own workload. Undertake duties in a timely manner whilst managing frequently competing demands on a daily basis. Have the ability to work under pressure within an agreed time frame Engage in clinical supervision and self-reflection to develop own professional standards and working in practices.

    PATIENT/CLIENT CARE Undertake assessments in accordance with the National Framework using clinical knowledge and expertise to assess patients health care needs anddetermine care requirements. Provide expert in relation to NHS Continuing Healthcare advice, support to a defined group of patients and liaise as appropriate with lead healthprofessionals. Have the ability to positively challenge and influence health and social care professionals to ensure that plans for patient care are appropriate and that the patient will be cared for on the most appropriate pathway where possible utilising the least restrictive options. Identify appropriate care provision and request provider assessments. Ensure that patients are given appropriate information in relation to Personal Health Budgets in accordance with NHS GM Policy. Undertake patient reviews in a timely manner ensuring that this includes a review of patient care, care records and care provision to ensurecommissioned care is appropriate, of acceptable quality and meets thepatients identified care needs. Act appropriately when there are requirements to re-consider care provision. Make recommendations of changes to provision and interim solutions if required. Have involvement in safeguarding investigations for NHS funded clients.Provide support to patients and consult with other practitioners andassociated staff to ensure high quality care.

    POLICY & SERVICE DEVELOPMENT Ensure National and local processes appertaining to NHS ContinuingHealthcare funding and NHS funded Nursing Care are adhered to. Contribute to NHS GM policy & procedure development and implementamendments to practice Actively participate with locality multi-disciplinary meetings to improveservices and improve outcomes for patients including the monitoring ofQuality within providers.

    FINANCIAL & PHYSICAL RESOURCES Ensure fair and consistent application of the NHS National Framework. Contribute to budget management by making sound clinical decisions. Ensure that providers are offering cost effective quality services Accountable for the appropriate use NHS Resources

    PEOPLE MANAGEMENT Maintain up to date knowledge and competence in line with professional and service requirements and demonstrate critical thinking, decision making and reflective skills to ensure own professional development. Provide expert advice and support on matters relating to NHS ContinuingHealthcare and Funded Nursing Care. Undertaking a statutory duty of care for your own personal safety and that others.

    Contribute towards the effective running of the service and establish effective liaison with all members of the Individualised Commissioning Team and the wider health and social care economy, supporting and deputising for colleagues as required. Provide support and leadership to student nurses on placement.

    RESEARCH, DEVELOPMENT & AUDIT Participate in the audit and evaluation of processes regarding the implantation of the national framework for Continuing Healthcare and Funded Nursing Care. Critically appraise available/current research and apply this knowledge tocurrent practice Contribute to service/policy development within NHS GM and partnerorganisations
