    Job summary

    As our team continues to develop its services in the communities that we serve, we are keen to appoint another team member to deliver high standards of holistic, individualised care and support to our patients and their families.

    As a Clinical Specialist you will be part of a multi-professional team, holding your own caseload and will work closely with health/social care professionals in the community, hospitals, and Acute Trust to ensure that patients requiring end of life care receive this in an integrated, timely manner.

    Main duties of the job

    You will also work closely with the Hospice at Home team and In Patient Unit and may on occasion provide cover to these services if required.

    We are seeking a Level 1 registered Nurse or registered Paramedic, with a minimum of three years post registration experience, preferably with experience gained within the community. Palliative care or oncology experience is essential. You should be able to work autonomously and manage a caseload, with support from the multidisciplinary team. A palliative care or oncology qualification and/or a non medical prescribing qualification would be desirable.

    Job responsibilities

    Effectively and efficiently manage a caseload of patients ensuring that specialist palliative is delivered in line with Isabel Hospice caseload management guidelines.

    Assess and manage symptom control and implement care plans with support from the Multidisciplinary Team and with the patients agreement.

    Give advice and support to patients and their families as required.

    Prescribe, if qualified to do so and authorised as a non medical prescriber with Isabel Hospice.

    Maintain and promote effective working and communication with other community professionals, hospital professionals, and care homes, voluntary, private and statutory agencies.

    Promote privacy, dignity and respect at all times.

    Cover the Duty CNS role on a roster basis.

    Recognise and report any safeguarding issues as per policy.

    Offer Level 2 psychological support to patients and carers as required.

    To be compliant with CQC and other regulatory bodies.

    Qualifications

    Essential

  • Level 1 registered nurse or registered paramedic with a minimum of three years post registration experience preferably with experience in the
  • community
  • Able to work autonomously and manage a caseload with support from the MDT
  • Palliative care or oncology qualification
  • Evidence of ongoing professional development
  • Desirable

  • A training/teaching qualification would be advantageous or willingness to work towards
  • Qualification or experience in mentoring/teaching would be an advantage
  • Non Medical Prescriber
  • Experience

    Essential

  • Palliative care/oncology experience
  • Experience of working autonomously and able to assess holistic needs and sign post to relevant services
  • An understanding of relevant legislation and practice relating to safeguarding and patient safety, MCA, DOLS and Childrens Act
  • Experience of working with imminently dying patients
  • Knowledge of health care policies and reforms related to palliative care and end of life agenda
  • Knowledge of CQC standards for hospice
  • Knowledge of principles of clinical governance and audit
  • Confident teaching skills
  • Good communication skills, both written and verbal and ability to use electronic patient record system
  • Excellent communication and interpersonal skills and the ability to manage conflict
  • Holistic assessment skills
  • Car driver and access to own vehicle and willingness to travel to varying work locations
  • Able to build effective working relationships
