About the Role:


Leighton-Linslade Health Connections Primary Care Network (PCN) is looking to recruit a paramedic, on a full-time basis, to work within our PCN multidisciplinary healthcare team.

Come and share your ideas with us, and use your experience to develop new services to support our member practices in the healthcare of our population.


The main responsibilities of the role include:

  • assessing and triaging patients, providing definitive treatment or making necessary referrals;
- advising patients on general healthcare, promoting self-management where appropriate, including signposting patients to other community or voluntary services;
- performing specialist health checks and reviews;
- interpretting ECGs;
- conducting investigatory procedures;
- collecting pathological specimens including, intravenous blood samples, swabs, etc.;
- supporting the delivery of anticipatory care plans and leading certain community services, such as monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing.

The postholder will work with a diverse range of people, from different cultural and social backgrounds. The ability to work confidently and effectively in a varied, and sometimes challenging, environment is essential.
- possess a BSc in a training programme approved by the College of Paramedics, or equivalent experience.
- possess Health and Care Professions Council (HCPC) registration.
- be able to operate at an advanced level of clinical practice.
- have the ability to make sound judgements in the absence of full information and manage varying degrees of risk when there is complex, competing, or ambiguous information or uncertainty.
- have experience of managing a designated caseload using theoretical and practical experience.
- have experience of completing assessments, planning & implementing interventions, and evaluating outcomes aligned to care plans.
- have excellent interpersonal and organisational skills
- have excellent communication skills.

Leighton-Linslade Health Connections is the primary care network for Leighton Buzzard and works with the three practices within the town, serving a population of circa 51,000.

We offer various primary care support roles to help the population take ownership of their health and connect with the community; working with the Personalised Care Model to help patients achieve their goals and focusing on what is important to them.


Our PCN team comprises an operations manager, clinical pharmacists, a paramedic, a physiotherapist, health & wellbeing coaches and care coordinators.
