    Job summary

    We are a GP Practice in Swindon, looking to recruit an enthusiastic and motivated Nurse Practitioner. We have 3 surgery sites in Swindon and you will be required to work at all a growing practice population, we are keen to continue to develop our clinical team.

    Main duties of the job

    We are looking for someone with excellent communication skills and a commitment to working as part of a strong and effective clinical team. We are flexible in terms of hours and a competitive salary will be offered to reflect the successful candidates experience and qualifications.

    Whalebridge Practice is looking for a Practice Nurse to join their multi-disciplinary team, to work over 3 sites within Swindon.

    The role can be part time or full time and the rate of remuneration will be based on qualifications and experience.

    Job Summary/Main Purpose

    Thismulti-site position is that of a Nurse Practitioner within the WhalebridgePractice. The practice serves thepopulation of Central and West Swindon and offers services across three siteswithin Swindon. We offer a wide range ofclinics and appointments to include chronic disease management, cervicalcytology, immunisations of babies and adults, and contraceptive services acrossseveral sites.

    Principal Duties

    To see, diagnose and treat patients with minor ailments.

    To provide assessment and clinical decision making using skilledevidence based knowledge working without supervision.

    To carry out home visits to patients who are acutely unwell.

    To carry out e-consultations, using the PATCHs system.

    IndividualResponsibilities

    Toassess, make clinical decisions and treat, using skilled evidence based nursingknowledge

    Touse effective communication in all situations which may necessitate the use ofa telephone interpretation service

    Torefer patients to other health care professionals as appropriate and necessary

    Toprovide advice on screening and health promotion at every opportunity

    Torecord all information accurately in conjunction with local policy and currentprofessional guidelines

    To work inpartnership with other members of the multidisciplinary team

    Tosupervise junior members of the team

    Tomentor students during placements, facilitate programmes for learners

    Tomaintain clinical/professional expertise to a high standard in accordance withNMC Code of Conduct

    Extendprofessional practice and competencies

    To participate instaff appraisal

    To act ethicallyto meet the needs of the patient

    Educationand Development

    Toparticipate in staff development, personal development planning and clinicalsupervision

    To take part inthe appraisal and revalidation processes.

    Participate in Researchprogrammes and explore opportunities undertake research to enhance the body ofknowledge in this field of nursing

    Assist with auditprogrammes

    Toattend training to develop and maintain skill.

    Tokeep registration up to date

    Toassist in the development andimplementation of policies/procedures and guidance relating to nursing practice

    To keep abreast ofprofessional developments, both locally and nationally

    GENERALRESPONSIBILITIES

    Confidentiality

    Youshall comply with the General Data Protection Regulations when processingpersonal data for patients and staff.

    Youmust be aware of the Caldicott Principles as they affect your work. You can address any questions in relation tothis to your line manager.

    Anydata breaches should be reported immediately to the Data Protection Officer.

    All enquiries from themedia must be referred to a Director.

    Health and Safety

    All employees have astatutory duty of care for the safety of themselves and others. Employees are required to report any hazardsas soon as they are identified.

    All staff must contribute to the management ofrisk. This requires them to assess therisk in all situations, take of appropriate actions to mitigate any risksidentified and report all hazards, incidents and near misses.

    All building occupiedby staff of TWGare non-smoking environments. No-one may smoke or vape in thebuilding. Staff who smoke or vapeoutside the buildings should cover their uniform.

  • 2 years practicing in Primary Care/Walk-in Centre/Community/A&E
  • Experience of multi-disciplinary working
  • Able to demonstrate high level of personal professional autonomy and accountability
  • Work using advanced clinical skills
  • Desirable

  • General Practice experience
  • Clinical knowledge and skills

    Essential

  • Work autonomously have the skills and knowledge to receive patients with undifferentiated and undiagnosed illnesses and injuries
  • Competent clinical skills with a knowledge of chronic disease management
  • Able to use analytical skills in practice
  • Excellent interpersonal and organisation skills
  • Immunisations experience
  • Clear understanding of legal issues, professional accountability and responsibility
  • Recognise and work within own scope of practice
  • IT experience
  • Excellent written and verbal communication skills
  • Experience in liaising with General Practitioners and other Health Care Professionals
