Clinical Service Lead - Community Transformation

A permanent opportunity has arisen to support with the community transformation in Southwark.

The successful applicant will work closely with the other 8a Clinical Service leads across the community pathways. Your focus will be on supporting staff through the transformation changes and contributing to staff development.

You will also support the development of the interface between the primary care mental health teams (PCMHT), long term community teams and other partner community teams such as Assertive Outreach Rehab, Early intervention and the Crisis teams.

This post is a great opportunity to develop or build on existing change management skills.

You will need to be innovative, dynamic and able to work autonomously.

Main Responsibilities

The Clinical Service Lead will support the implementation of transformation process. The role involves working across all the community teams in Southwark.

The post holder will lead on allocated tasks required for the implementation of the transformation.

This will enable the community teams to continue to provide maximise effectiveness and efficiency of service provision throughout the ongoing transformation.

The aim is to ensure integrated clinical, operational, quality and financial outcomes are achieved within the service line.

Our Trust headquarters is located at Denmark Hill less than 5 minutes from the train station (zone 2) and is within walking distance from the beautiful green spaces of Ruskin park and the vibrant high-street that offers great shopping opportunities and with a wide range restaurants.

Quality

  • To performance manage the quality of the clinical services, including patient safety, patient satisfaction, clinical outcomes and compliance with the clinical pathways and associated protocols
  • To use and provide information required for the quality measure which will be collected & reported using dash boards to improve the quality of care delivery

Clinical effectiveness

  • To provide demonstrable assurance that the resources available achieve efficiencies, reduction of waste and improvements in clinical quality.

  • To promote the engagement of carers and families in the care pathway.
  • Responsible for ensuring high levels of operational effectiveness, including throughput, resource utilisation and the attainment of performance targets within service lines.
  • To work in partnership with housing providers, faith organisations day centre staff, outreach teams and other agencies providing support to the client groups
Leadership

  • To support the development of a culture that promotes leadership through coaching
  • To assist in recruiting, motivating, training, developing and retaining appropriate staff enabling them to have the skills, expertise and discretion to function effectively in their roles
  • Provide demonstrable assurance that staff have the appropriate skills and competence to deliver high quality care
  • To demonstrate clinical competence and role model effective clinical care delivery
  • To recruit/develop team leaders/managers who meet the organisational standards and measure performance against leadership and management competencies adopted by the Trust
