An exciting opportunity has arisen for a Full Time Band 3 Support Time Recovery worker to work within the Warrington Later life and Memory Service Community Team.

This role is predominantly to provide carer support both face to face and over the telephone. There will also be a requirement to cover OPA clinic completing ECG's, physical observations, and memory assessments.

The operational hours are Monday to Friday 9am - 5pm and we are based on the Hollins Park


The LLAMS service is an accredited service which provides the comprehensive assessment and treatment of people with dementia; there is a small cohort of people who have a functional illness.

  • To assist in the assessment, planning, implementation and evaluation of programmes of care for service users, including health care assessments under the direction and supervision of qualified members of
  • To assist in the facilitation of medical review clinics, under the direction of qualified members of
  • Contribute towards reports required by managers or external agencies, for example child care conferences or court
  • Working as part of a multi-disciplinary team in a non-judgemental
  • Assisting in the facilitation of group work with service users, under the direction and supervision of qualified members of

  • Contribute towards the maintenance of accurate client records according to Trust
  • To liaise with staff from other agencies, taking into consideration confidentiality
  • To contribute to effective running of the service by providing and receiving information to/from other departments or services. Ensure this information is provided in the most effective way i.e. telephone, fax or in
  • To assist service users with self-administration of oral medication and ensure any concerns/issues about medication are reported back to a qualified
  • To use a variety of verbal and non-verbal skills to overcome communication barriers presented by service users due to their mental health
  • To develop and maintain links with other agencies and services, both statutory and voluntary.
  • To use skills and techniques to de-escalate situations where service users become physically or verbally
  • To contribute to the development of risk management plans for service users by reporting to the qualified practitioner any issues or concerns regarding service users behaviour/action towards themselves or
  • To adhere to the Trust's rights based approach taking into account service user
  • To take responsibility for ensuring your own personal safety and that of the service user when using
  • To take reasonable care for the health and safety of yourself and any others that may be affected by your own acts and omissions at
  • To ensure that you are familiar with the terms and conditions or your

  • To participate in the services' performance management supervision framework and work towards meeting identified goals and objectives as agreed in personal development
