Description
Holistic Assessment and Personalised Care:
Conduct holistic assessments and co-design Health and Well-being Plans, focusing on individual needs and preferences. Provide continuity of care and be the point of contact throughout the social prescription process.
Collaborative Engagement and Training:
Build relationships with GP practices and attend multi-disciplinary team meetings. Train and develop GPs and healthcare teams on identifying suitable patients for social prescribing.
Community Connection and Support:
Encourage self-referrals and connect with underrepresented communities. Help individuals address wider issues impacting their health and well-being. Co-produce personalized support plans and connect individuals to relevant groups and services.
Stakeholder Collaboration and Reporting:
Establish strong links with local organizations and attend relevant meetings.
Gather feedback from service users and referral agencies to assess quality and impact. Maintain comprehensive data and evaluation systems and provide regular reports.
Compliance and Professional Development:
Comply with safeguarding, confidentiality, and data protection protocols. Adhere to health and safety policies and promote equality, diversity, and ethical principles.
Work flexibly and carry out any reasonable duties as required by the service.
