    We have an exciting opportunity for a Band 6 Physiotherapist within the new Hounslow Community Neuro-rehabilitation Team and the unique Neuro Timely Supported Discharge Team to support our ability to meet the Integrated Community Stroke Service (ICSS) delivery model.

    Our Vacancy is for Senior Neuro Physiotherapist - Band hrs - Permanent)This is an ideal post for an experienced Band 5 physiotherapist looking to further develop their neurology and management skills or a current Band 6 looking for a new challenge.

    This job will allow the successful applicant to further develop a breadth of clinical skills with patients who often have a complex neuro presentation, as well as consolidate the necessary skills required for specialised practice.

    There is a strong focus on personal and service development, and you will be given opportunities to participate in in-service training programme and external training where appropriate.

    We also have highly experienced clinicians within the team who are keen to support your ongoing personal and professional development.

    The Hounslow Community Neuro Rehab Team (CNRT) is a client-centred, integrated multi-disciplinary team comprising of an Advanced Clinical Practitioner, Physiotherapists, Occupational Therapists, a Neuro Psychologist, a Pharmacist, a Specialist Nurse practitioner, an Occupational Therapy Assistant and Rehab assistants.

    The Neuro-rehab team works across both intensive short-term rehab and longer-term rehab pathways.

    There are opportunities to develop skills in rapid assessment and facilitating discharge from hospital, long term condition management and specialist neuro-rehab.

    The service has developed specialist MDT clinics for Parkinsons' Disease and Multiple Sclerosis and this post enables opportunities to develop specialist skills with these conditions.

    The team also help rehabilitate patients following acute stroke within the community in both pathways and treat patients with a variety of long-term neurological conditions.

    HRCH provides community health services for around 523,000 people registered with GPs in the London boroughs of Hounslow and Richmond, but also serves a wider population across southwest London for a range of more specialist services.

    Every day our professionals provide high-quality healthcare in people's homes and convenient local clinics.

    We help people to stay well in the community, manage their own health with the right support and avoid stays in hospital.

    We employ around 1,300 people, who work across a wide range of health centers, hospitals, GP surgeries, children's centers, local council facilities and in community settings - including in people's homes.

    Travel is an integral part of the post so applicants should possess the ability and willingness to travel across the borough for work purposes.

    A Valid HCPC registration Relevant post-graduate experience in the NHS as a Band 5/6 Physiotherapist including working with people with neurological conditions.

    Diploma/Degree in physiotherapy or equivalent
    Good theoretical and practical knowledge of adult and neurological rehabilitation treatments and modalities
    Understand the legal responsibilities of the profession

    Completed minimum 18 months as Band 5 physiotherapist
    Experience working in a neuro rehabilitation team

    Experience working within community rehabilitation physiotherapy services

    Experience of teaching other physiotherapy staff members, patients and other health care professionals
