    Job Description

    Thepost holder will work within their clinical competencies to providespecialist
    pharmaceutical supportto all practices of the PCN including working withmembers
    of the wider healthcareteam to improve care and support safe andeffective
    prescribing.
    Moving between practices the role will have a patient facingelement either face
    to face ortelephone including chronic disease management repeatprescription
    management andstructured medication reviews. This will also includethe
    management and review ofpatients with polypharmacy.
    Thepost holder will need to be proactively involved in qualityimprovement and
    audit as well asmanaging some aspects of the Quality and OutcomesFramework.
    There will also be therequirement for the post holder to contribute tothe
    provision of out ofhours/extended hours/improved accessservices.
    The post holder will besupported to help develop the role and work to improvethe
    care and outcomes for thepatients across Health west PCN.
    To play a pivotal role in the delivery of high quality primaryhealth care within
    Healthwest

    Requirements

    Clinicalmedication reviews for patients with single or multiple long termconditions where medicines optimisation is required To reconcilemedicines following discharge from hospitals or intermediate careand working with patients and community pharmacists to ensurepatients receive the medicines they need postdischarge and toreduce risk of readmission. To be involved in multidisciplinaryclinical meetings within the PCN where appropriate Contribute tothe provision of out of hours/on call/extended services for thepractice and the patients. These can include patient facing andtelephone consultations signposting to other services and/orhealthcare professionals where appropriate while working withinscope of practice and limits of competency To manage repeatprescription requests resolving queries where possible within scopeof practice. To ensure patients have appropriate monitoring testsin place when required. Implement changes to medicines in line withMHRA alerts product withdrawal or shortage and other local ornational guidance To maintain full and complete records of allpatient contacts using appropriate clinical templates andcoding
