We are recruiting for x2 (Band 3) Healthcare Assistants to join our new Early Onset Type 2 Diabetes Service and Diabetes 8 Care Team.


This role will work within the Diabetic 8 Care Team to undertake the 8 care process checks to those patients across Staffordshire who are at most in need and are at higher health risk of complications associated with diabetes.

This service will encompass the delivery of 8 care process checks, misclassification of Diabetes type, as well as advice and guidance for pregnancy and pre-pregnancy, psychological need, unmet social need and weight management.

Direct referral pathways will be set up for you to signpost/refer too, dependent on patient needs.


EOT2D clinics will be planned in community venues and clinic settings and will require clinics being offered during early evenings and Saturday mornings when required.


In addition, the role will support the Diabetes 8 Care Team in the delivery of 8 care process checks to all patients living with Type 1 and Type 2 who don't form part of the above service.

The role will involve delivery within clinic settings and community venues where applicable.

The role will involve close liaison with the Lead Nurse and Operational Lead for Diabetic 8 Care.

The 8 care process checks that the roles the postholder will be expected to undertake are as follow:
Blood Pressure - You will record on bespoke software the reading for blood pressure using a calibrated cuff

Cholesterol - Ensure a sample is taken in accordance with all health and safety and infection control polices and delivered to a testing facility on the same day

Serum Creatinine Ensure a sample is taken in accordance with all health and safety and infection control polices and delivered to a testing facility on the same day

Urine albumin - Ensure a sample is taken in accordance with all health and safety and infection control polices and delivered to a testing facility on the same day

Foot surveillance - Responsible for following the guidance and training for recording foot screening in line with protocol

BMI - patient height and weight

Smoking - Establish the smoking habits for the patient and record as appropriate

HbA1c - Take a blood sample in accordance to Phlebotomy protocols and delivered to ta testing facility on the same day

All policies and protocols will be followed for each of the tests above.

Follow and implement Trust, departmental and professional policies, procedures, guidelines and standards within own work area

Responsible for capturing patient details and recording results on the patient's record accurately and in a timely way.
